purpose of waiving the requirements of obtaining sealed bids for the sole
source purchase of a M -Vac System, to include three (3) years of preventative
maintenance and service, training and accessories from M -Vac Systems, Inc.,
located at 640 West Sandy Parkway, Sandy, Utah 84070.
Inquiries from other potential sources of such a package who feel that they
might be able to satisfy the City's requirements for this item you may contact
Pearl Bethel, Senior Procurement Contracting Manager, at the City of Miami
Procurement Department at (305) 416-1949.
The Miami City Commission requests all interested parties be present or
represented at the meeting and may be heard with respect to any proposition
before the City Commission in which the City Commission may take action.
Should any person desire to appeal any decision of the City Commission
with respect to any matter to be considered at this meeting, that person shall
ensure that a verbatim record of the proceedings is made including all testimony

Said proposed resolution may be inspected by the public at the Office of the City Clerk, 3500

Commission Meeting.

The Miami City Commission will hold a Public Hearing on Thursday, July
25, 2019 at 9:00 A.M., to consider the award of a contract to the foreign
limited liability company listed below through Anti -Poverty grant funds from
the District 5's share of the City of Miami's Anti -Poverty Initiative Program.
Jessie Trice Community Health System, Inc., a Florida Not for Profit
Corporation, for Oral Health Initiative, and to consider the City Manager's
recommendations and finding that competitive negotiation methods are
not practicable or advantageous regarding these issues:
• Jessie Trice Community Health System, Inc. - Oral Health Initiative
Inquiries regarding this notice may be addressed to Malissa T. Sutherland,
Administrative Assistant ll, Office of Grants Administration, at (305) 416-1005.
This action is being considered pursuant to Section 18-85 (A) of the Code
of the City of Miami, Florida as amended (the "Code"). The recommendations
and findings to be considered in this matter are set forth in the proposed
resolution and in Code Section 18-85 (A), which are deemed to be incorporated
by reference herein and are available as with the regularly scheduled City
Commission meeting of July 25, 2019 at Miami City Hall, 3500 Pan American
Drive, Miami, Florida.
The Miami City Commission requests all interested parties be present or
represented at the meeting and may be heard with respect to any proposition
before the City Commission in which the City Commission may take action.
Should any person desire to appeal any decision of the City Commission
with respect to any matter to be considered at this meeting, that person
shall ensure that a verbatim record of the proceedings is made including all
