The grant was awarded to provide outreach, information, referral, assessment and placement services to homeless individuals discharged from the Miami -Dade County's judicial court and public health systems. Background of Item: The Miami -Dade County Homeless Trust, recognizing the need for intervention to prevent those discharged from the criminal justice and public health systems from becoming homeless, established the Memorandum of Agreement grant to fund a service provider to provide a range of services designed to address the needs of the temporarily and chronically homeless. The Homeless Trust, decided to extend the contract for additional twelve (12) months to the Miami Homeless Assistance Program as the grantee, to provide outreach, housing assistance, and coordinate with the jail, prison, hospital, foster care, mental health and emergency room facilities to provide appropriate interventions. City of Miami ` City Hall * 9fi 11 IM&0Q:AT10 '® Legislation 3500 Pan American Drive Miami, FL 33133 Resolution www.miamigov.com Enactment Number: R-19-0090 File Number: 5424 Final Action Date:3/14/2019 A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S), ESTABLISHING A SPECIAL REVENUE PROJECT TITLED "MEMORANDUM OF AGREEMENT (MOA) PROGRAM, PC -1819 -MOA" AND APPROPRIATING FUNDS IN AN AMOUNT NOT TO EXCEED OF THREE HUNDRED FORTY THOUSAND DOLLARS ($340,000.00) CONSISTING OF A GRANT FROM MIAMI-DADE COUNTY THROUGH THE MIAMI-DADE COUNTY HOMELESS TRUST ("COUNTY"), WITH NO CITY OF MIAMI ("CITY") MATCHING FUNDS REQUIRED, FOR THE PROVISION OF INTERVENTION SERVICES FOR THE TEMPORARILY AND CHRONICALLY HOMELESS AS WELL AS THE PROVISION OF HOUSING, TRANSPORTATION, AND OTHER SUPPORTIVE SERVICES FOR INDIVIDUALS REFERRED THROUGH THE MIAMI-DADE COUNTY JUDICIAL AND HEALTH SYSTEMS, WITH SAID SERVICES TO BE PROVIDED BY THE CITY'S DEPARTMENT OF HUMAN SERVICES' ("DEPARTMENT") HOMELESS SERVICES DIVISION ("GRANT"); AUTHORIZING THE CITY MANAGER TO EXECUTE A GRANT AGREEMENT WITH THE COUNTY FOR SAID PURPOSE, IN SUBSTANTIALLY THE ATTACHED FORM ("AGREEMENT"), CONDITIONED UPON THE AVAILABILITY OF THE CITY RESOURCES THAT MAY BE REQUIRED TO IMPLEMENT THE ACCEPTANCE OF AND COMPLIANCE WITH SAID GRANT; FURTHER AUTHORIZING THE CITY MANAGER TO EXECUTE ANY AND ALL AMENDMENTS TO THE AGREEMENT OR DOCUMENTS NECESSARY, ALL IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, IN FURTHERANCE OF THE GRANT. WHEREAS, the City of Miami ("City") has been awarded a grant in an amount not to exceed of Three Hundred Forty Thousand Dollars ($340,000.00) from Miami -Dade County through the Miami -Dade County Homeless Trust ("County") for the operation of the City's Department of Human Services' ("Department") Homeless Services Division for Fiscal Year 2018-2019 ("Grant"); and WHEREAS, the Grant will be used for the provision of intervention services for the temporarily and chronically homeless as well as the provision of housing, transportation, and other supportive services, as necessary, with no City matching funds required; and WHEREAS, it is appropriate for the City Manager to enter into a Grant Agreement with the County for acceptance of the Grant for said purpose, in substantially the attached form ("Agreement"), and to establish a new special revenue project for the appropriation of the Grant; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. The City Manager is authorized to execute the Agreement, in substantially the attached form, between the County and the City accepting the Grant, with no City matching funds required, for the provision of intervention services for the temporarily and chronically homeless as well as the provision of housing, transportation, and other supportive services, as necessary, with said services to be provided by the Department's Homeless Services Division conditioned upon the availability of the City resources that may be required to implement the acceptance of and compliance with said Grant. Section 3. The following new Special Revenue Project is hereby established with resources for the same appropriated as described below: PROJECT TITLE: Memorandum of Agreement (MOA) Program, PC -1819 -MOA RESOURCES: The Miami -Dade County Homeless Trust $340,000.00 APPROPRIATIONS: Provision of intervention services $340,000.00 for the temporarily and chronically homeless as well as the provision of housing, transportation, and other supportive services, as necessary. Section 4. The City Manager is further authorized to negotiate and execute any and all amendments to the Agreement and documents necessary, all in a form acceptable to the City Attorney, in furtherance of the Grant.
