A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
AUTHORIZING THE CITY MANAGER TO ACCEPT AND APPROPRIATE A
TOTAL AMOUNT OF $918,000.00 IN THE FORM OF ONE (1) REIMBURSEMENT
GRANT FROM THE STATE OF FLORIDA ("FLORIDA") AGENCY FOR
PERSONS WITH DISABILITIES ("APD") TO THE CITY OF MIAMI ("CITY')
DEPARTMENT OF PARKS AND RECREATION ("PARKS") FOR HOME AND
COMMUNITY-BASED SERVICES AND FAMILY AND SUPPORTED LIVING
PROGRAMS ("PROGRAMS") FOR PERSONS WITH DISABILITIES;
ESTABLISHING A NEW SPECIAL REVENUE PROJECT, ENTITLED
"PROGRAMS FOR THE DEVELOPMENTALLY DISABLED 2017-2022," TO FUND
SERVICES AND PROGRAMS FOR THE DEVELOPMENTALLY DISABLED
ADMINISTERED BY PARKS; AUTHORIZING THE CITY MANAGER TO CREATE
A NEW PROJECT NUMBER FOR THE PROGRAM, TO CREATE THE
NECESSARY ACCOUNTING STRUCTURE TO ACCOUNT FOR THE GRANT
FUNDS IN ACCORDANCE WITH GENERALLY ACCEPTED ACCOUNTING
PRINCIPLES AND TO COMPLY WITH THE GUIDELINES SET FORTH BY APD
AS GRANTOR; FURTHER AUTHORIZING THE CITY MANAGER TO EXECUTE
A MEDICAID WAIVER PROVIDER RENEWAL AGREEMENT ("AGREEMENT")
WITH THE FLORIDA ADP, IN SUBSTANTIALLY THE ATTACHED FORM; AND
TO NEGOTIATE AND EXECUTE ALL OTHER NECESSARY DOCUMENTS,
AMENDMENTS, EXTENSIONS, RENEWALS, AND MODIFICATIONS TO THE
AGREEMENT, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, TO
IMPLEMENT THE ACCEPTANCE OF AND COMPLIANCE WITH THE GRANT
AWARD FOR SAID PURPOSES, FOR THE PERIOD COMMENCING JANUARY
1, 2017 THROUGH DECEMBER 30, 2022.
WHEREAS, the City of Miami ("City"), through the Department of Parks and Recreation
("Parks"), obtains funds to support its Home and Community -Based Services ("HCBS") and
Family and Supported Living ("FSL") programs through the State of Florida ("Florida") Agency
for Persons with Disabilities ("APD"); and
WHEREAS, APD is providing funds to the City, on a reimbursement basis, in the
estimated amount of $918,000.00 for a five (5) year period commencing January 1, 2017
through December 30, 2022, for HCBS and FSL services and programs provided within the City
to developmentally disabled individuals; and
WHEREAS, the City Manager and Parks are requesting that the City Commission
authorize the City Manager to negotiate and execute a new Medicaid Waiver Service Renewal
Agreement ("Agreement"), in substantially the attached form, for the period of January 1, 2017
through December 30, 2022;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:

Section 1. The recitals and findings contained in the Preamble of this Resolution are
adopted by reference and incorporated as if fully set forth in this Section.
Section 2. The City Manager is authorized' to accept and appropriate a total amount of
$918,000.00 in the form of one (1) reimbursement grant from the Florida APD to Parks for
persons with disabilities, and to establish a new Special Revenue Project, entitled "Programs for
the Developmentally Disabled 2017-2022," to fund services and programs for the
developmentally disabled administered by Parks.
Section 3. The City Manager is further authorized' to create a new project number for
the Program, to create the necessary accounting structure to account for the Grant funds in
accordance with Generally Accepted Accounting Practices and to comply with the guidelines set
forth by APD as grantor.
Section 4. The City Manager is authorized' to execute the Agreement, in substantially
the attached form, and to negotiate and execute all other necessary documents, amendments,
extensions, renewals, and modifications to the Agreement, in a form acceptable to the City
Attorney, for compliance with the Agreement and to implement the acceptance of and
compliance with the grant award for said purposes, for the period commencing January 1, 2017
through December 30, 2022.
Section 5. This Resolution shall become effective immediately upon its adoption and
signature of the Mayor.2

' The herein authorization is further subject to compliance with all requirements that may be imposed by
the City Attorney including but not limited to those prescribed by applicable City Charter and City Code
provisions.
2 If the Mayor does not sign this Resolution, it shall become effective at the end of ten (10) calendar days
from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
