ALFONSO TYPE OF BENEFIT PROPOSED BENEFITS Effective March 4, 2414 SALARY $199,000 annual salary payable bi-weekly (To remain in effect until changed by the City Commission.) Car Allowance $800 per month I, Cellular Phone Allowance $200 per month i INSURANCE HealthNision/Dental City paid premium for Executive Benefit Plan City paid premium for 2X Annual Salary Life Accidental Death & City paid premium for 2X Annual Salary Dismemberment Insurance Long Term/Short Term Disability Insurance City pays 100% of premium. EXECUTIVE PHYSICAL One annual physical LEAVE TIME Vacation 20 days per year, with a maxium of 15 days to be taken consecutively Sick 12 days per year Holidays 11 days per year Upon honorable separation, City Manager shall receive no payment of any unused Vacation and Sick leave. SEVERANCE The City Manager may be removed pursuant to the process set forth in the City Charter. If the City Manager is removed without cause, he shall receive a fixed sum for compensation and forbearance in an amount equivalent to one months' salary and benefits upon a written full release of in favor of the City. The City Manager shall not receive any severance if he is terminated for cause, including, but not limited to, misconduct, neglect of duty, misfeasance, or malfeasance or if he voluntarily resigns. PENSION The City Manager elects to participate in a retirement plan as provided under Sec.40-351 (1)(d) of the Code. The City shall contribute at the rate of 20% of the City Manager's annual salary. I DEFERRED COMPENSATION1 -0- COLA Same as granted to the members of the classified or unclassified service, whichever is higher.
