Sawyer Plaza Assisted Living Facility History Concerned with the lack of affordable services that would allow the elderly to "age in place", Miami -Dade County, through its Public Housing and Community Development, converted the public housing development Helen M. Sawyer Plaza into an assisted living facility (ALF) in 1998. The Helen M. Sawyer Plaza ALF is the first facility of its kind to be transformed from public housing. Helen M. Sawyer Plaza ALF is a multi -service facility providing a continuum -of -care for low- income elderly residents who wish to maintain their independence without worrying about difficulties with one or more of the essential activities of daily living. The property was named after Helen M. Sawyer, a former housing agency employee who was known as a humanitarian. The Facility The facility, built in 1976, is licensed under Miami -Dade County and is comprised of 104 units in an eight -story high-rise building, covering 2.38 acres of land. There are 83 efficiencies and 21 one -bedroom units. The property is part of PHCDs continuing initiative to provide housing for low-income residents who require minimal assistance to live with dignity and independence. In the Neighborhood The Helen M. Sawyer Plaza ALF is accessible to public transportation. The property is within walking distance of the Culmer Metrorail station and provides easy access for families who wish to visit their loved ones. The facility is also near Jackson Memorial Hospital, the Veterans Medical Center, Bascom Palmer Eye Institute, and the University of Miami Sylvester Cancer Care Center. A major supermarket and shopping centers are also located close by. Amenities The Helen M. Sawyer Plaza ALF provides a range of amenities and services enabling the elderly residents to age with dignity. These amenities include: accessible telephones on every floor, cable TV, community room, air conditioning, emergency alarm systems, and housekeeping services. Other amenities included: • 24-hour staffing • Private living quarters • On-site food preparation facilities that provide three meals and three snacks daily • Community dining area • Transportation to cultural and recreational events (supervised activities and field trips) • Secure Facility/Electronic entry system • Coordination of transportation to medical appointments (escort provided if necessary) Submitted into the public recorl for itM(s) t on t 2 City Clerk Eligibility Requirements Residents of Helen M. Sawyer Plaza ALF must be: • Elderly • Medicaid -eligible • Able to perform daily living activities with supervision or assistance • At -risk of being prematurely placed in a nursing home In addition residents must not require 24-hour medical attention, licensed professional mental health treatment, must not be bedridden, and must be free from communicable diseases. Cost The State of Florida, through its Department of Elder Affairs provides cash assistance to eligible individuals through its Optional State Supplementation (OSS) program. Its purpose is to supplement a person's income to help pay for costs in an assisted living facility, mental health residential treatment facility, and adult family care home.
