AGENDA ITEM SUMMARY FORM CONTINUED
Pursuant to Resolution No. 15-0068, adopted on February 26, 2015, the Miami City Commission
approved the allocation of ESG funds in the total amount of $399,538 which included an amount of
$138,907.60 to Citrus Health Network, Inc. for Rapid Rehousing & Homeless Prevention Program
and Administration for the period of April 1, 2015 to March 31, 2016.
Citrus Health Network, Inc. started spending these funds from October 1, 2015 due to an extension
of the 2014-2015 fund to September 30, 2015.
The extension of the time for the agreement will allow Citrus Health Network, Inc., to expend program
year 2015-2016 allocations for a program that is very much needed in the community.
The Administration recommends granting Citrus Health Network, Inc., an extension of time until
September 30th, 2016 to spend the funds for Rehousing & Homeless Prevention Program including
program administration.
