Ladies and Gentlemen:
The City 'of Miami Plat and Street Committee, at its meeting of December 4, 2014. approved the above
tentative plat subject to the following revisions being inade to the tentative Plat, additional information
being provided and/or variances being granted. Please be advised that the processing of your tentative plat
cannot proceed until these conditions have been satisfied:

a) Correct Subdivision names of the underlying plat and surrounding subdivisions.
b) Provide Zoning designations and zoning transect lines.
On application
a) Provide a Certificate of Good Standing for all entities on tile chain of ownership.
b) Application for Forbes Miami NEI" Avenue LLC, must: be notarized.
Provide a line type for the Monument line different than the center line. See the City of Miami
Municipal Atlas Sheet for th ' e coranionly,used line type.
Show existing underground storm sewer information located. within the right of way.
Provide a copy of the traffic study.
Obtain and. provide confirmation from the Planning and Zoning Department that the proposed final
plat is consistent with Miami2l., Appendix D regulations, inclusive of "Miami World Center"
-Development, Standards, prior to filial plat submittal..
Obtain and. provide confirmation from the Planning and Zoning Department that the proposed filial
plat. is consistent with the "Development Agreement", inclusive of any amendments thereto entered
into by and between the Owner and the City of Miami, prior final plat submittal.
Provide a proposed development site plan to the, City's Police and Fire — Rescue Departments prior
to final plat submittal.
Provide verification of tentative plat approval from Miami. — Dade County.
The traffic study to be submitted for this tentative plat must address the comments and conditions
required by the Miami — Dade County Public Works/Waste Management Department,
Miami Parking Authority review and. approval is required for removal of the on -street parking
spaces.
1.2. Show the, existing drainage system on private property to determine if there are any connections to
the public storm sewer system located. in the right-of-way.

15. Show the existing property line (limit of the plat) as a solid line and the proposed right-of-way line
as a dashed line. Be advised that OD the final plat this will be reversed.
16. On main sketch, show underlying lots with dimensions.
17. After the legal description, add the statement "AN EXPRESS PURPOSE OF THIS PLAT IS TO
CLOSE, VACATE AND A13ANDON FROM PUBLIC USE THAT PORTION OF T14E STREET
LYING WITHIN'THE, LIMITS OF THIS PLAT, and then add the legal description of the R/W to
be dedicated.
18. Provide a copy of the Certified Corner Record.
19. In the title block under the tentative plat name provide a brief and correct legal. description, also
include section, township, range, city, county and state..
20. Provide an updated Boundary Survey less than a year old..
21. Sho-,,%, the Lot and Block numbers, Subdivision names, and Plat Book and Page numbers of the
surrounding subdivisions.
22. Correct the dedication of right of way (Gross hatch) at the ends of NE, 7 Street.
23, Indicate which. structures are to remain, if any, and those that are to be removed.
24. In the surveyor's notes, indicate "Boundary and Topographic Survey".
25. A fetter from Comcast is required to determine if any adjustments to their facilities or easements are
required.
26. Verify with the Miami -Dade County that tb.e proposed plat name is acceptable,
27. All encroachments across proposed tract and/or lot lines must be removed prior to final plat
submittal,
28. An opinion of title, in the City of Miami opinion of title form, must be, provided at the time of the
final plat submittal.
29. Current backup documentation will be required for all who execute the final plat. A. resolution for
authority to execute documents and a. Certificate of Good Standing from the Secretary of State are
required, if applicable.
30. Tentative plat application must be made with. Miami -Dade County after receiving approval from the
City of Miami. Plat and Street Committee,
31. Be advised that an incomplete final plat package will not be accepted by the City of Miami. It is the
owner's responsibility to work with his surveyor and his attorney to assure that everything is in,
order before submitting the final. plat package,
32. Be advised that if all the requirements for scheduling the final plat for City Commission action are
not in. order, the final plat will. not be scheduled for a City Commission meeting.
33. A letter of concurrency from the Miami — Dade County School Board is required prior to the final
plat approval (see contact sheet).
34. A letter from the Miami -Dade Water and Sewer Department is required to determine if any
adjustments to their facilities or easements are required.
35. Provide a letter requesting the withdrawal of the Miami World Center Plat I -Tentative Plat #1820-
A approved at the December 5, 2013 Plat and Street Committee meeting.
In addition to the above requireinGnts, you should be aware of the following:
State and local laws require the installation of various physical improvements in the public
rights -of way when, property is platted. These subdivision in1prOVeftlelAS include paving,
drainage., landscaping, sidewalks, etc. In some cases this could represent a substantial
investment on your part.
REQUIRED: The submission of an electronic copy of the tentative plat, in an AutoCAD
version, to the City of Miami Public Works Department, Roadway Plans Section will
EXPEDITE the preparation and enhance the ACCURACY of the subdivision improvement
letter required for the final plat, approval.

2. The alteration, relocation or installation of utilities such as storm and sanitary sewers, electric,
telephone, water, etc., caused by this plat- will be at the property owner's expense. Also, utility
easements may be required on the property being platted. until the final plat is recorded
3. A building permit. will not be issued on the property being platted
or as authorized by the City of Miami Code Section 55-10(i). Also, the Certificate of
Occupancy for any building construction will be issued only after all the required subdivision
improvements have been completed.
4. Approval for fire flow requirements must be obtained from the Fire -Rescue Department prior
to the issuance of a building permit.
5. In order to mitigate probleni.s associated with access and construction activities throughout the
neighborhood, the contractor/developer is encouraged to notify the affected neighborhood
residents, in writing, of the project starting and completion dates at the titne of issuance of
permits. The contractor/developer may coordinate the notification of residents with the local.
City of Miami NET Service Center.
6. Additional items must be provided to the City of Miami Department of Public Works before
the final plat, is submitted to the City Conimission for approval.. You will be notified in writing
as to what these items are after the amount of the bond has been detennined for the necessary
subdivision improvements.
