A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
AUTHORIZING THE CITY MANAGER TO ACCEPT A GRANT AWARD IN A TOTAL
AMOUNT NOT TO EXCEED $141,000.00 ("GRANT") FROM THE HEALTH
FOUNDATION OF SOUTH FLORIDA ("HFSF") FOR THE ADMINISTRATION AND
COORDINATION OF HFSF'S LIVE HEALTHY COMMUNITY PARTNERSHIP
INITIATIVE IN LITTLE HAVANA ("PROGRAM") FROM OCTOBER 1, 2015
THROUGH DECEMBER 31, 2016; ESTABLISHING AND APPROPRIATING THE
GRANT FROM A NEW SPECIAL REVENUE PROJECT FUND ENTITLED "LIVE
HEALTHY LITTLE HAVANA INITIATIVE" IN ORDER TO FUND THE PROGRAM;
FURTHER AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE
THE NECESSARY DOCUMENTS, AMENDMENTS, EXTENSIONS, RENEWALS,
AND MODIFICATIONS, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, IN
ORDER TO IMPLEMENT THE ACCEPTANCE OF AND COMPLIANCE WITH SAID
GRANT AWARD, WITH THE OPTION TO RENEW FOR UP TO FOUR (4)
ADDITIONAL ONE (1) YEAR TERMS AT A NOT TO EXCEED AMOUNT OF
$120,000.00, UNDER THE SAME TERMS AND CONDITIONS.
WHEREAS, in February 2014, the Health Foundation of South Florida ("HFSF") launched an
initiative aimed at strengthening community capacity to collaboratively plan, and collectively carryout
strategies to improve health; and
WHEREAS, after an extensive selection process, HFSF selected the neighborhood of Little
Havana to invest up to $3.75 million over a six (6) year period; and
WHEREAS, "root causes" of health issues in Little Havana have been identified based on an
assessment of available data and input from community residents, which resulted in a comprehensive
Community Action Plan aimed at addressing those root causes that have been prioritized; and
WHEREAS, the City of Miami ("City") shall be responsible for the administration and coordination
of the Live Healthy Little Havana Initiative ("Program") to ensure broad, multi -agency and resident
representation in the initiative; and
WHEREAS, the City Manager is requesting authorization to both accept a grant award in a total
amount not to exceed $141,000.00 from the HFSF for the administration and coordination of the
Program from October 1, 2015 through December 31, 2016 and to establish and appropriate the grant
from a new Special Revenue Project Fund entitled "Live Healthy Little Havana Initiative" in order to
fund the Program;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,
FLORIDA:
Section 1. The recitals and findings contained in the Preamble of this Resolution are adopted by
reference and incorporated as if fully set forth in this Section.

Section 2. The City Manager is authorized to accept a grant award in a total amount not to exceed
$141,000.00 from the HFSF for the administration and coordination of the Program from October 1,
2015 through December 31, 2016.
Section 3. The following new Special Revenues Project Fund is established, and resources are
appropriated as described herein:

$141,000.00
APPROPRIATIONS: Live Healthy Little Havana Initiative $141,000.00
Section 4. The City Manager is further authorized {1} to negotiate and execute the necessary
documents, amendments, extensions, renewals, and modifications, in a form acceptable to the City
Attorney, in order to implement the acceptance of and compliance with said grant award, with the
option to renew for up to four (4) additional one (1) year terms at a not to exceed amount of
$120,000.00, under the same terms and conditions.
Section 5. This Resolution shall become effective immediately upon its adoption and signature of
the Mayor. {2}

{1} The herein authorization is further subject to compliance with all requirements that may be imposed
by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code
provisions.
{2) If the Mayor does not sign this Resolution, it shall become effective at the end of ten (10) calendar
days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become
effective immediately upon override of the veto by the City Commission.
