A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
AUTHORIZING THE CITY MANAGER TO ACCEPT A GRANT AWARD IN A
TOTAL AMOUNT NOT TO EXCEED $295,969.00 ("GRANT") FROM THE
HEALTH FOUNDATION OF SOUTH FLORIDA ("HFSF") TO CONTINUE THE
ADMINISTRATION AND COORDINATION OF HFSF'S LIVE HEALTHY
COMMUNITY PARTNERSHIP INITIATIVE IN LITTLE HAVANA ("PROGRAM")
WHICH TOTAL GRANT FUNDS (1) SHALL NOT EXCEED $144,375.00 FOR THE
PROGRAM YEAR FROM JANUARY 1, 2018 THROUGH DECEMBER 31, 2018
("2018 PROGRAM YEAR") AND (2) SUBJECT TO AVAILABILITY OF FUNDS
AND SUCCESSFUL COMPLETION OF 2018 GOALS AND OBJECTIVES, SHALL
NOT EXCEED $151,594.00 FOR THE PROGRAM YEAR FROM JANUARY 1,
2019 THROUGH DECEMBER 31, 2019 ("2019 PROGRAM YEAR");
ESTABLISHING AND APPROPRIATING THE GRANT FROM A NEW SPECIAL
REVENUE PROJECT FUND ENTITLED "LIVE HEALTHY LITTLE HAVANA
INITIATIVE 2018-2019" IN ORDER TO FUND THE PROGRAM; FURTHER
AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE THE
NECESSARY DOCUMENTS, AMENDMENTS, EXTENSIONS, RENEWALS, AND
MODIFICATIONS, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, IN
ORDER TO IMPLEMENT THE ACCEPTANCE OF AND COMPLIANCE WITH
SAID GRANT AWARD AND THE OPTION TO RENEW FOR THE 2019
PROGRAM YEAR.
WHEREAS, in February 2014, the Health Foundation of South Florida ("HFSF")
launched an initiative aimed at strengthening community capacity to collaboratively plan and
collectively carryout strategies to improve health; and
WHEREAS, after an extensive selection process, HFSF selected the neighborhood of
Little Havana to invest up to $3.75 million over a six (6) year period; and
WHEREAS, "root causes" of health issues in Little Havana have been identified based
on an assessment of available data and input from community residents, which resulted in a
comprehensive Community Action Plan aimed at addressing those root causes that have been
prioritized; and
WHEREAS, since 2015, the City of Miami ("City") has been responsible for the
administration and coordination of the Live Healthy Little Havana Initiative ("Program") to ensure
broad, multi -agency, and resident representation in the initiative; and
WHEREAS, following the adoption of Resolution No. 15-0491, the City executed a
Memorandum of Understanding with HFSF on January 14, 2016 and a renewal thereof on
January 12, 2017; and
WHEREAS, the cost of the Program has increased, HFSF has requested for the City to
begin a transition to become the Host Agency, and HFSF wishes to allocate additional funding
to cover anticipated Program related expenses (1) in an amount not to exceed $144,375.00 for

the Program year from January 1, 2018 through December 31, 2018 ("2018 Program Year") and
(2) subject to funds availability and successful completion of previous year goals and objectives
for a one-year renewal, in an amount not to exceed $151,594.00 for the Program year from
January 1,2019 through December 31, 2019 ("2019 Program Year"), for a total new grant award
amount of not to exceed $295,969.00; and
WHEREAS, the Administration is requesting authorization (1) to accept a grant award in
a total amount not to exceed $295,969.00 from the HFSF for the administration and
coordination of the Program and (2) to establish and appropriate the grant from a new Special
Revenue Project Fund entitled "Live Healthy Little Havana Initiative 2018-2019" in order to fund
the Program;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the Preamble of this Resolution are
adopted by reference and incorporated as if fully set forth in this Section.
Section 2. The City Manager is authorized' to accept a grant award in a total amount
not to exceed $295,969.00 from the HFSF to continue the administration and coordination of the
Program, which total grant funds (1) shall not exceed $144,375.00 for the 2018 Program Year
and (2) subject to availability of funds and successful completion of 2018 goals and objectives,
shall not exceed $151,594.00 for the 2019 Program Year.
Section 3. The following new Special Revenues Project Fund is established and
resources are appropriated as described herein:

Florida 2019 Program Year
APPROPRIATIONS: Live Healthy Little Havana Initiative $295,969.00
Section 4. The City Manager is further authorized' to negotiate and execute the
necessary documents, amendments, extensions, renewals, and modifications, in a form
acceptable to the City Attorney, in order to implement the acceptance of and compliance with
said grant award, with the option to renew for the 2019 Program Year.
Section 5. This Resolution shall become effective immediately upon its adoption and
signature of the Mayor.2
' The herein authorization is further subject to compliance with all requirements that may be imposed by
the City Attorney, including but not limited to, those prescribed by applicable City Charter and City Code
provisions.
2 If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from
the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
