A RESOLUTION AUTHORIZING THE CITY
MANAGER TO EXECUTE AN AGREEMENT, IN
SUBSTANTIALLY THE ATTACHED FORM,
WITH FAMILY HEALTH CENTER, INC. FOR
PROFESSIONAL SERVICES IN THE FORM
OF PHYSICAL EXAMINATIONS FOR
CURRENT AND PROSPECTIVE EMPLOYEES
FOR THE PERIOD ENDING JULY 31,
1988; ALLOCATING FUNDS THEREFOR IN
AN AMOUNT NOT TO EXCEED $62,800
FROM CURRENTLY APPROPRIATED FUNDS.
WHEREAS, this service will be used by the Department of
Personnel Management for the purpose of providing physical
examinations for current and prospective employees for the period
ending July 31, 1988; and
WHEREAS, funds for this service are available from currently
appropriated funds; and -
WHEREAS, the City Manager and the Director of the Department
of Personnel Management recommend that the execution of this
agreement be approved;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
- Section I. The City Manager is hereby authorized to execute
an agreement in substantially the attached form with Family
Health Center, Inc. for professional services in the form of
physical examinations for current and prospective employees
through the period ending July 31, 1988 at a total proposed cost
not to exceed $62,800 allocating funds thereof from currently
appropriated funds as follows:
a) From the Self -Insurance and Insurance Trust Fund fiscal
f
year budget 1987 - 1988, an amount not to exceed $36,920, of
which $18,920 is from the Claims Division account and $18,000
from the Group Health Division account;
b) From the Salaries account, Personnel Management
Department fiscal year budget 1987 - 1988, an amount not to
exceed $25,800.

ENCLOSURES.
It is respectfully recommended that the City Commission adopt the
attached resolution authorizing the execution of an agreement
with Family Health Center, Inc., in substantially the form
attached, for professional services in the form of physical
examinations for current and prospective City of Miami employees
at a total cost not to exceed $62,800 for the period ending July
31, 1988.
BACKGROUND:
Section 2-268 of the Charter and Code of the City of Miami
mandates the Department of Personnel Management to establish and
maintain medical services which shall include a medical
examination of employees and potential employees. In order to
fulfill this requirement, the Department of Personnel Management
has analyzed the current contract utilized by Metropolitan Dade
County for the purpose of providing medical assessment and
testing of employees. The contract was analyzed because it
contains a special provision which enables governmental units in
Metropolitan Dade County to avail themselves of this contract.
Based on the analysis, it is recommended that the City of Miami
enter into an agreement with the low bidder, Family Health
Center, Inc., for the purpose of providing physical examinations
of current and prospective employees.
Funds are currently allocated in the fiscal year 1987-88 budget
for this service.
It is estimated the contracting out of these services will result
in a savings of $89,507 for the period (December 23, 1987 - July
31, 1988).

This letter serves to officially inform you of the City of
Miami's intent to enter into an agreement with Family Health
Center, Inc., for the purpose of providing medical services to
current and prospective City of Miami employees. Our intent is
pursuant to the special provision in the "Dade County Employee's
Medical Assessment and Testing" contract (Bid No. 1087 - 7/31/87-
OTR-CW) which allows any governmental unit in Metropolitan Dade
County to avail themselves of this contract.
The City of Miami would like for the Family Health Center to
provide the following medical services:
1. Conduct pre -employment physicals on City of Miami
applicants.
2. Conduct promotional physical examinations.
3. Conduct annual physical examinations on approximately 30
designated employees.
4. Conduct physical examinations of employees returning to
work from personal illness and/or line of duty injury.
NOTE: Examination protocol for items 1, 2, and 3
outlined in Attachment A.
To ensure proper and current insurance coverage, please provide
certificates of insurance on the coverages listed below as
required by the original bid.

A. Worker's Compensation Insurance for all employees as
required by Florida Statute 440.
D. Public Liability Insurance on a comprehensive basis in
an amount not less than $300,000 combined single limit
plus occurrence for bodily injury and property damage.
City of Miami must be shown as additional insured with
respect to this coverage.
C. Automobile Liability Insurance covering all owned, non -
owned and hired vehicles used in connection with the
work, in an amount not less than $300,000 combined
single limit her occurrence for bodily injury and
property damage.
D. Proof of Medical Malpractice Insurance.
The following procedures have been established to facilitate the
coordination of medical services between the City of Miami anti
Family Health Center, Inc.
A. Scheduling
The hours of 9:00 a.m. - 10:00 a.m., Tuesday through
Friday have been designated by your staff as City of.
Miami time slots for approximately 5-7 patients.
Additional slots will be made available on an .is needed
basis.
B. Examination Results
Examination results will be available within three (3)
working days of the physical and will be picked up by
the City of Miami.
