Purpose or use of Funds
Funding assistance is requested to rehabilitate and preserve a 10-unit multifamily affordable housing
property currently subject to a City of Miami Unsafe Structures case and recertification compliance
requirements. Funds will be utilized for engineering, permitting, structural rehabilitation, electrical
upgrades, life -safety improvements, restoration of vacant units, and activities necessary to obtain
final recertification approval.

PURPOSE OF ADDENDUM
This Addendum supplements the Preliminary Funding Application and Redevelopment
Proposal previously submitted to the Omni Community Redevelopment Agency for the
preservation and rehabilitation of the multifamily residential property located at 1731 NW
1 st Place, Miami, Florida.
The purpose of this Addendum is to further define the ownership team's proposed
affordability commitment and Area Median Income (AMI) unit mix in support of the Omni
CRA's affordable housing preservation and community revitalization objectives.
AFFORDABLE HOUSING PRESERVATION COMMITMENT
Ownership recognizes the importance of preserving affordable housing opportunities
within the Omni CRA district and is committed to maintaining long-term housing
affordability while ensuring the financial sustainability of the property following completion
of the proposed rehabilitation.
Subject to funding approval, final program requirements, and execution of a funding
agreement acceptable to all parties, ownership is willing to commit to a twenty (20) year
affordability period for the residential units located at the property.
JAM ESTOWN
This commitment is intended to preserve housing opportunities for extremely low-income,
very low-income, low-income, and workforce households while supporting the long-term
viability of the property as a safe, secure, and professionally managed housing asset.
PROPOSED AMI UNIT MIX
Ownership proposes the following affordability structure:

AFFORDABILITY IMPACT
The proposed affordability structure results in:
• 20% of units reserved at 40% AMI
• 60% of units reserved at or below 50% AMI
• 80% of units reserved at or below 60% AMI
• 1 00% of units reserved at or below 80%AMI
This structure provides a balanced approach that maximizes affordability while
maintaining sufficient operating revenue to support property maintenance, insurance,
reserves, management, and long-term capital needs.
COMMUNITY BENEFITS
The proposed affordability commitment will:
• Preserve ten affordable housing units within the Omni CRA district
• Expand housing opportunities for extremely low-income households
• Support workforce housing needs
• Improve housing stability for residents
• Prevent the loss of existing affordable housing inventory
• Support neighborhood revitalization efforts
• Enhance resident safety through substantial rehabilitation and modernization
improvements
• Ensure long-term affordability for future residents
LONG-TERM PRESERVATION STRATEGY
The ownership team's goal is not merely to rehabilitate the property but to establish a long-
term affordable housing asset that serves the community for decades to come.
The proposed twenty-year affordability commitment reflects ownership's willingness to
partner with the Omni CRA to create lasting community benefit through housing
preservation, neighborhood stabilization, resident safety improvements, energy -efficient
modernization, and responsible property stewardship.
Ownership believes the proposed affordability structure aligns with the mission and
objectives of the Omni Community Redevelopment Agency and demonstrates a
meaningful commitment to preserving affordable housing opportunities within the
community.
REQUEST FOR CONSIDERATION
Ownership respectfully requests that the Omni CRA consider this affordability
commitment and AMI structure as part of its evaluation of the funding application and
redevelopment proposal for 1731 NW 1st Place.
The ownership team looks forward to working collaboratively with the Omni CRA to

hereby authorize Jamestown General Contractors, LLC and its representative, Marcus A. James,
to act on my behalf regarding matters related to the building recertification, rehabilitation,
permitting, and compliance process for the above -referenced property.
This authorization includes, but is not limited to, the authority to:
1. Communicate with the City of Miami, Miami -Dade County, the Unsafe Structures
Division, Building Department personnel, inspectors, plan reviewers, and other
governmental agencies regarding the property.
2. Obtain information, records, reports, inspection results, permit records, violation records,
code compliance information, hearing notices, and other documents related to the
property.
3. Coordinate with architects, engineers, surveyors, consultants, contractors, subcontractors,
utility providers, and other professionals involved in the recertification and rehabilitation
process.
4. Attend meetings, hearings, inspections, and conferences on my behalf, including hearings
before the City of Miami Unsafe Structures Panel.
5. Prepare, submit, and retrieve applications, plans, reports, supporting documentation,
permit packages, recertification documents, and related materials necessary to advance
the recertification and compliance process,
6. Communicate with and submit documentation to the Overtown Community
Redevelopment Agency (CRA) and other funding organizations regarding potential
funding opportunities for the property.
7. Request status updates and receive information related to permit reviews, code
compliance matters, recertification requirements, and project progress.

permitting, engineering coordination, and rehabilitation activities for the above -referenced
property.
This Letter of Authorization does not grant authority to transfer ownership, execute deeds,
mortgage the property, obtain financing in the Owner's name, sign construction contracts on
behalf of the Owner, or otherwise convey any ownership interest in the property.
This authorization shall remain in effect until revoked in writing by the Owner.

2. Apply for, obtain, execute, and sign permits, applications, approvals, inspections,
certificates, notices, and related documents concerning repairs, renovations, maintenance,
construction, financing, insurance claims, and code compliance matters.
3. Obtain, review, request, and sign documents related to construction, permitting,
inspections, contractor proposals, estimates, project management, and rehabilitation
activities.
4. Access the Property and coordinate repairs, improvements, maintenance, construction
activities, inspections, and contractor scheduling.
5. Meet with lenders, inspectors, insurance adjusters, appraisers, surveyors, engineers,
municipal officials, and code enforcement representatives concerning the Property.
6. Receive and review correspondence, notices, violations, utility information, code
enforcement matters, and property -related documentation.
7. Request and obtain records, plans, permits, surveys, inspections, utility records, and
other documents concerning the Property.
8. Execute construction -related administrative documents necessary to facilitate repairs,
rehabilitation, permitting, inspections, and financing activities.
9. Represent the Principal before Miami -Dade County, the City of Miami, utility providers,
insurance companies, lenders, and contractors concerning matters affecting the Property.
10. Coordinate and manage all renovation, rehabilitation, repair, maintenance, inspection,
permitting, and financing activities related to the Property.
LIMITATIONS
This Power of Attorney is limited solely to matters concerning the Property located at 1776
NW 41st Street, Miami, Florida 33142.
The Agent shall NOT have authority to:
• Sell, transfer, convey, or gift the Property.
• Execute a deed transferring ownership.
• Create or transfer ownership interests.
• Execute a mortgage or lien against the Property unless specifically authorized in a
separate written instrument.
• Change' estate planning documents or beneficiary designations.
DURABLE POWER
This Durable Power of Attorney is not terminated by the subsequent incapacity of the
Principal as provided under Chapter 709, Florida Statutes.
EFFECTIVE DATE AND TERMINATION
This Power of Attorney shall become effective immediately upon execution and shall remain
in effect until:
• Completion of all rehabilitation, financing, permitting, inspection, and related activities
concerning the Property; or
• Written revocation by the Principal; or
• December 31, 2027,
