A RESOLUTION ACCEPTING THE BID RECEIVED
PROM NORTH AMERICAN LIFE AND CASUALTY COMPANY
ON SEPTEMBER 23, 1974, PROVIDING AN
ACCIDENTAL DEATH INSURANCE IN THE AMOUNT
OF $20,000.00 FOR CITY OF MIAMI EMPLOYEES
FOR THE PERIOD FROM OCTOBER 1, 1974
THROUGH SEPTEMBER 30, 1975 AT AN ANNUAL
PREMIUM OF $26,985.60 , WITH AN OPTION
TO RENEW BY MUTUAL CONSENT OF BOTH
PARTIES FOR AN ADDITIONAL TWO (2) YEARS:
AND AUTHORIZING THE CITY MANAGER TO INSTRUCT
THE PROPER CITY OFFICIALS TO IMPLEMENT THIS
PROGRAM.
WHEREAS, pursuant to Public Notice, sealed bids were
received September 23, 1974 providing an accidental death
insurance in the amount of $20,000.00 for City of Miami
employees for the period from October 1, 1974 through Septem-
ber 30, 1975, with an option to renew by mutual consent of
both parties for an additional two (2) years; and
WHEREAS,
invitations
were mailed to fifty-six
potential suppliers and eight bids were received; and
WHEREAS, the City Manager recommends that the award
be made to NORTH AMERICAN LIFE AND CASUALTY COMPANY which is the most
advantageous bid to the city;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION
OF THE CITY OF MIAMI, FLORIDA:
Section 1. The bid received from NORTH AMERICAN
LIFE AND CASUALTY COMPANY on September 23, 1974, providing
an accidental death insurance in the amount of $20,000.00
for City of Miami employees for the period from October 1,
1974 through September 30, 1975, with an option to renew by
mutual consent of both parties for an additional two (2) years
be, and is hereby, accepted at an annual premium -of $26,985.60.

Section 2. The City Manager be, and he is hereby,
authorized to instruct the proper City Officials to implement
this program.
