INC. AS SERVICING AGENT OF EQUITABLE
LIFE ASSURANCE SOCIETY OF THE UNITED
STATES FOR THAT CARRIER'S GROUP, LIFE
AND HEALTH INSURANCE COVERAGE OF CITY
OF MIAMI EMPLOYEES FOR WHICH PAYMENT
IS TO BE MADE BY THE CARRIER OF THE
NON -LEVEL SERVICING AGENT'S COMMISSION
BASED UPON THE PREMIUM FOR THE PERIOD
COMMENCING OCTOBER 1, 1976 AND PROVID
ING FOR THE WITHDRAWAL OF SAID RECOGNI-
TION UPON 30 DAYS NOTICE TO THE CARRIER
IN WRITING.
WHEREAS, the Equitable Life Assurance Society
the United States continues to provide group, life and
health insurance to City of Miami employees with the renewal
effective October 1, 1976; and
WHEREAS, the firm of Edwin H. Cole, C.L.U. &
Associates, Inc. has previously been recognized by the City
Commission in Resolution No. 76-283 as servicing agent for
such coverage;
NOW, THEREFORE, BE IT RESOLVED BYr�THE UCOM1�4ISSION ,_ x
OF THE CITY OF MIAMI, FLORIDA: ITEM NO.
Section 1. The firm of Edwin H. Cole, C.L.U.
Associates, Inc. is hereby recognized as servicing agent of the
Equitable Life Assurance Society of the United States for which
he is to be paid the non -level servicing agent's commission
by said company based upon the premium for the period commenc-
ing October 1, 1976, with the right reserved to withdraw
herein recognition upon 30 days notice to the carrier in
