COMPLETE SETTLEMENT OF ANY AND ALL CLAIMS
AND DEMANDS AGAINST THE CITY OF MIAMI, UPON
THE EXECU£ION OF A RELEASE, RELEASING THE

EREAS, Mr. Walter Lee Turner and Sarah Turner,

ainst the City of Miami for alleged bodily injury, resulting
rom an accident on August 10, 1975► at or near N.W. 46th Stree

WHEREAS, the above claim has been investigated by the
Division of the City Attorney's office in accordance with
finance No. 8417, which creates the City of Miami's Self -Insurance
Program, and said office recommends that this claim be settled
or, the sum of $7,250.00;

the City of Miami, upon the execution of a release, releasing

Walter Lee Turner has filed a claim against the City of Miami
through his Attorney, Anthony Capodilupo, for a bodily injury
Claim resulting from an accident involving a City Police vehicle.
This individual normally resides in Alabama and therefore does not
have any personal injury protection coverage available.
This accident occurred on August 10, 1975, at 10:38 A.M., on N.tv.
46th Street, 45 feet west of the intersection with N.E. 12th Avenue,
Miami, Florida.
The vehicle in which Mr. Turner was a passenger, was facing west
on 46th Street, stopped for a red light. The Police vehicle struck
the rear of the claimant vehicle, resulting in $511.58 damage to
the unit.. The Police vehicle suffered $483.03 damage. Approximately
15 feet of skid marks were left previous to the point of impact. A
brake failure was evident, and this was supported by the personnel
at the City Motor Pool.

the City Attorney's office and in accordance with Ordinance No. 8417,
which creates the City of Miami Self --insurance Program, and said
