ROSILLO, IHER ATTORNEY, WITHOUT THE AD- MISSION OF LIABILITY, TIIE SUM OF $1,200.00 IN FULL AND COMPLETE SETTLEMENT OF ALL BODILY INJURY CLAIMS AND DEMANDS AGAINST THE CITY OF MIAMI, AND UPON EXECUTION OF A RELEASE, RE- LEASING THE CITY OF MIAMI FROM ALL CLAIMS AND DEMANDS, AND TO PAY TO TIIE UNITED STATES FIDELITY & GUARANTY COMPANY, WITHOUT THE ADMISSION OF LIABILITY, THE SUM OF $100.00 IN FULL AND COMPLETE SETTLEMENT OF TIHEIR PERSONAL INJURY PROTD2TION LIEN, CLAIM AND DEMAND AGAINST TIIE CITY OF MIAMI, AND UPON THE EXECUTION OF A RELEASE, RELEASING THE CITY OF MIAMI FROIM9 ALL CLAIMS AND DEMANDS. WIHEREAS, MARINA MOLINA, through her attorney, Albert P. Rosillo , filed a claim against the City of Miami for the alleged bodily injury claim, and United States Fidelity & Guaranty Company filed a claim against the City of Miami for alleged personal injury protection lien, resulting from a fall down incident on February 2, 1976, at or near 501 N.W. 30th St., Miami, Florida; and WHEREAS, the above claim has been investigated by the Torts Division of the City Attorney's Office, in accordance with Ordinance No. 8417, which creates the City of Miami's Self In- surance Program and said office recommends that these claims be settled for the sum of $1,200.00 and $100.00 respectively; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF "DOCUMEN?)( THE CITY OF MIAMI, FLORIDA: Section 1. That the Director of Finance is91-1- ITEM f, authorized to pay to Marina Molina, her husband , Jorge, and Albert P. Rosillo, her attorney, without the admission of liability, the sum of $1,200 in full and complete settlement of CITY COMMISSION MEETING OF RESOLUTION NO.!...? 4' RE1iARKS: aii bodily injury and property damage claims and demands against the City of Miami, upon the execution of a release, releasing the City of Miami from all bodily injury and property damage claims and demands. Section 2. That the Director of Finance is hereby authorized to pay to United States Fidelity & Guaranty Company, without the admission of liability, the sum of $100.00 in full and complete settlement of the personal injury protection lien, claims and demand against the City of Miami, upon the execution of a release, releasing the City of Miami from all personal injury protection liens, claims and demands. IKnox, City ' torney October 12, 1977 VC-76-041 r MOLINA, Marina vs. CITY OF MIAMI :E.<,D/A: 2/2/76 P/A: In front of 501 NW 30th St. Miami, Florida Resolution Marina Molina, filed a claim against the City of Miami, through her attorney, Albert P. Rosillo, Law Office, involving bodily injury and a personal injury protection lien from the United States Fidelity & Guaranty Company, resulting from a fall down incident involving a City of Miami sidewalk, a junction of the pavement and driveway on 501 Northwest 30th Street, Miami, Fla. This incident occurred on February 2nd, 1976, on 501 N.W. 12th Avenue, Miami, Florida. Marina Molina stepped into a depression outside an apartment house, causing her injuries to her face and nose (fracture). The injury is diagnosed as nasal fracture and dental damages. The special damage material is as follows: Doctors Dr. Hernandez Dr. Pinero Dates of Treatment , Amount 2-2-76 thru 5-76 $1,458.00 3-2-76 thru 5-76 150.00 'total Special Damages: 1,608.00 The above claim has been investigated by the Torts Division of the City Attorney's Office and in accordance with Ordinance No. 8471 which creates the City of Miami Self -Insurance Program and said claims be settled for $1,200.00 to include the bodily injury claim presented by Marina Molina and $100.00 in settlement of the personal protection lien of United States Fidelity & Guaranty Company.
