LHANC will provide an estimated 18,525 Congregate Meals to an estimated 75 clients daily. The center is located in District 1, but service is available to all City of Miami residents. Program start Date: 12/01 /2025 Program End Date: 11/30/2026 Please describe how this program/project and funding will alleviate poverty within the City of Miami? LHANC will provide a nutritious meal to older adults, 60 years or age or older. This meal will assist these older adults who may not have money and access to a full nutritious meal, free of charge, Sy participating in this program, clients also have access to additional meals offered to those clients who are at the highest risk for malnutrition through other funding sources. IMPACT AND PERFORMANCE: Describe the overall expected outcomes and performance measures for this project/program. And, Ilst the supporting documentation that will be submitted with the Close Out Report: 66% of the seniors who receive a meal will maintain or improve their nutrition status. This is measured by comparing the client's nutrition score on their initial assessment to the nutrition score on their annual reassessment. LHANC uses the form established by the Florida Department of Eider Affairs ("DOEA"), At the close of the program year, LHANC will provide documentation of all clients who received meals, the total number of meals served and billed, the payments to the caterer for the meals ordered and any other documents requested by the City of Miami.
