for alleged hodily injury, resulting from an accident on May 17,
1977, at or
near N.W. 6th Street and N.W. 1:;t Avenue.
WHEREAS, the above claim has been investigated by the Torts

No. t1•I17, which creates the City of Miami's Self -Insurance Program,

Settlement of bodily injury
Claim of Evidio Ruiz
Mr. Evidio Ruiz ha:, filed a bodily injury claim against the City of
through his attorney, 'l'homas J. Gaine, of Gaine & Gaine P.A.
This accident ocarrcj on Mav 17, 1977, at or near N.W. 6th St.and
N.W. 1st Avenue, Miami, Florida.
The City of Miami Police vehicle was northbound on First Ave., which
is controlled by a :stop sign at the intersection of N.W. 6th St.,
which is a one-way street westbound.
The Police vehicle proceeded thru the intersection and was involved
in a collision with the claimant vehicle which was traveling west
on N.W. 6th Street.
Mr. Evidio 'Zuiz, was the driver of the claimant vehicle, receiving
acute cervical, thoracic and iumho: acral spine sprain and strain OL
the Para: r in al musculature of the regions, also contusion and r•cch::-mo:;i:.
of the right thigh anterior aspect.
i'hi.: individual lost 4 weeks and 2 days of work, which is verified.
The total medical expense is$2, 781.8`5, while the lost time and wage's
