Ladies and Gentlemen:
The City of Miami Plat and Street Committee at its meeting of April 6, 2023 approved the above tentative
plat subject to the following revisions being made to the tentative plat, additional information being
provided and/or variances being granted. Please be advised that the processing of your tentative plat cannot
proceed until these conditions and/or comments have been satisfied.
Resilience and Public Works
1. Location sketch needs revisions.
2. Tentative plat needs general revisions. Contact the Survey Section.
3. Right of way dimension on NE 2 Avenue does not match City records (Municipal Atlas Sheet
shows 25ft and recorded Plat Book 2, Page 21 shows 35ft).
4. Submit a formal request to the Supervisor of Plats to eliminate the requirements for a rounded
corner radius pursuant to Section 55-11(a)(6) of the City Code, at the S.E. corner of N.E. 2 Avenue
and N.E. 61 Street.
5. Provide corrections to underground information of existing stormwater drainage system adjacent to
proposed plat — underground lines, inlet rim elevation, inlet type, structure bottom elevation, pipe
invert elevation and direction, pipe size, pipe length, exfiltration trench. All information obtained
from the City records must be field verified. Field verifications are also required for all new
structures that are not on the City records. All structures must be cleaned to be accessible for the
field verifications and researches. County and City systems must be separated. Provide missing
underground lines for stormwater drainage system.
6. For coordination of underground information for stormwater drainage system, contact Danay
Lizazo or dlizazo@miamigov.com, Development and Roadway Plans Section, for comments.
7. Corrections on underground information to be completed prior to the submittal of SDI engineering
plans.
8. Surveyor's Notes #7 must be revised.
9. Submit a formal notification letter cancelling and abandoning MCID 2 tentative plat #2006 and
#1996 (formerly #1974-B).

Planning/Zoning
10. Obtain confirmation if the existing Unity of title(s) and/or Covenant(s) running with the land listed
on the Opinion of Title are to be released. Contact Alicia Menardy ATMenardy@miamigov.com
11. The density provided exceeds the maximum number permitted. Indicate on the plat the correct density
and the reason for any additional density. Contact Terran Steed TSteed@miamigov.com
12. The DuPois Medical Office and Drugstore building located at the NW corner of the plat is an
individually designated historic resource. Magic City Innovation SAP development agreement,
condition #15. The developer agrees to ensure the preservation of the historically designated

13. An application for a Special Certificate of Appropriateness (SCOA) has been submitted for
demolition of the Dupuis Medical Office and Drugstore located at 6041 NE 2 Av. The SCOA has
been scheduled for review by the Historic and Environmental Preservation Board on May 2, 2023.
Revise the tentative plat to state that the building is to be removed. Contact Anna Pernas,
Preservation Officer, Historic Preservation Office, apernas@miamigov.com
14. Confirm that the Special Exceptions (lthrough 22) listed on the tentative plat have been verified by
the Planning Department. Contact Maria Adefeya, Planner II, Planning Department at
madeyefa@mamigov.com
Standard Comments
15. Provide an updated tentative plat survey with the fmal plat submittal.
16. A letter from Comcast is required to determine if any adjustments to their facilities or easements are
required.
17. Verify with Miami -Dade County that the proposed plat name is acceptable.
18. All encroachments across proposed tract and/or lot lines, must be removed prior to final plat
submittal.
19. An Opinion of Title, in the City of Miami Opinion of Title form, must be provided at the time of
final plat submittal.
20. Current backup documentation will be required for all who execute the final plat. A resolution for
authority to execute documents and a Certificate of Good Standing from the Secretary of State are
required, if applicable.
21. Tentative plat application must be made with Miami -Dade County after receiving approval from the
City of Miami Plat and Street Committee. Provide a copy of the Notice of Action to RPW Survey
Section.
22. Be advised that an incomplete final plat package will not be accepted by the City of Miami. It is the
owner's responsibility to work with his/her surveyor and attorney to assure that everything is in
order before submitting the final plat package.
23. Be advised that if all requirements for scheduling the final plat for City Commission action are not
in order, the final plat will not be scheduled for a City Commission meeting.
24. A letter of concurrency from Miami -Dade County School Board is required prior to final plat
submittal.
25. A letter from Miami -Dade Water and Sewer Department is required to determine if any adjustments
to their facilities or easements are required. Also, a letter to release the fmal plat in connection with
water and sewer service agreements.
26. Provide two 8 %" x 11" paper location sketch exhibits, one "Before Replat" and one "After Replat"
location sketch exhibits.

In addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements in the public rights -
of -way when property is platted. These subdivision improvements include paving, drainage,
landscaping, sidewalks, etc. In some cases, this could represent a substantial investment on your
part. REQUIRED: The submission of an electronic file of the tentative plat, in an AutoCAD, to the
City of Miami Resilience and Public Works Department, Development and Roadway Plans Section
will EXPEDITE the preparation and enhance the ACCURACY of the subdivision improvement
letter required for the final plat approval.
2. Execution of Covenant for Postponement of Subdivision Improvements is required in cases where
the required subdivision improvements are not completed.
3. The alteration, relocation or installation of utilities such as storm and sanitary sewers, electric,
telephone, water, etc., caused by this plat will be at the property owner's expense. Also, utility
easements may be required on the property being platted.
4. A building permit will not be issued on the property being platted until the final plat is recorded or
as authorized by the City of Miami Code Section 55-10(i). A certificate of use may be issued for
existing buildings prior to subdivision improvements being completed as per the Development
Agreement for the SAP.
5. Approval for fire flow requirements must be obtained from the Fire -Rescue Department prior to the
issuance of a building permit.
6. In order to mitigate problems associated with access and construction activities throughout the
neighborhood, the contractor/developer is encouraged to notify the affected neighborhood residents,
in writing, of the project starting and completion dates at the time of issuance of permits.
7. Additional items must be provided to the City of Miami Department of Resilience and Public
Works, Development and Roadway Plans Section before the final plat is submitted to the City
Commission for approval. You will be notified in writing as to what these items are after the
amount of the bond has been determined for the necessary subdivision improvements. It is required
that the applicant contacts the City of Miami Resilience and Public Works Survey Section to verify
that all final plat submittal documents are complete prior to final plat submittal.
8. Tentative plat approval is only valid for one (1) year and six (6) months from the date of the Plat
and Street Committee meeting at which time it was approved.
