TENTATIVE PLAT #2040
LOCATED AT THE SOUTH SIDE OF N.W. 73 STREET BETWEEN N.W. 1 COURT AND N.W.
MIAMI PLACE.
Ladies and gentlemen:
The City of Miami Plat and Street Committee, at its meeting of November 2, 2023, approved the above
tentative plat, subject to the following revisions being made to the tentative plat, additional information
being provided and/or variances granted. Please be advised that the processing of your tentative plat cannot
proceed until these conditions have been satisfied.
Resilience and Public Works:
1. The Application needs revisions in items #4, #9, and in page 2.
2. Revise the Opinion of Title, page 3, to be in City of Miami Opinion of Title form.
3. Only show the legal description of record title.
4. Verify if Other Exceptions #2, 3, and 4 affect the subject property, if not, remove them.
5. Provide a revised Written Consent in a format acceptable to the City Attorney's Office.
6. All companies in the signature block must be registered in the State of Florida.
7. On the Tentative Plat remove the legal description by the surveyor and only show the legal
description of record title.
8. Add "Boundary and Topographic" survey to the certification.
9. The digital signature is overlapping the signature block. Re-sign in the appropriate space, and mark
the check box for the applicable surveyor's name.
10. The Location Map needs revisions, label the subject property with the name of the tentative plat,
and replace "Proposed Tract "A" with "Unplatted".
11. The fractional description is incomplete, revise accordingly.
12. In the main sketch, correct the label of the point of beginning.
13. Add ties to block comers and street center line and monument line intersections, with bearings and
distances.
14. Label the "zoned center line".
15. Revise "proposed 25' right-of-way" to "right-of-way to be dedicated by this plat".
16. Revise the line types to be in City of Miami standard.
17. Remove the folio numbers and recording information from the main sketch.
18. Remove or revise Surveyor's Notes #7 and 10, and remove #16.
19. The tentative plat needs additional revisions. Contact the Survey Section.

Development and Roadway and Plans:
20. Provide corrections on the underground information of existing storm water drainage system
adjacent to proposed plat — underground lines, inlet rim elevation, inlet type, structure bottom
elevation, pipe invert elevation and direction, pipe size, pipe length, pipe material, exfiltration
trench, and legend. The complete underground stormwater drainage utilities are to be located,
whether shown in City records or not. Obstructed structures must be cleaned to be accessible for
field verification.
21. For coordination of underground information for stormwater drainage system, contact Mauricio
Valdes at 305-416-1288 or MauValdes@miamigov.com, DRPW - Development and Roadway
Plans Section for comments. To obtain copy of Stormwater As-Builts, Capital Projects, and any
other underground City's Records, contact Justo De La Torre at jdelatorreAmiainigov.com.
22. Surveyor's Notes #6 needs to be revised to indicate that all information was field verified.
23. Corrections on underground information to be completed prior to Subdivision Improvement (SDI)
engineering plans submittal.
Planning/Zoning:
24. The Location Map needs revisions, add Transect lines and Zoning labels, no dash in D 1.
25. Revise Surveyor's Notes #14, Current Zoning District should be "D1" Work Place District Transect
Zone.
26. A "Tree Table" is required, certified by an Arborist, including name, title, license number and the
date of the Tree Survey.
27. The Planning Department's Land Development's review fee of $295.00 needs to be paid. For
information contact Ricardo Lopez-Aldana at rlopezaldana@miamigov.com or (305)-416-1601.
28. Add the area (in square feet) of proposed Tract "A" to the main sketch and to the Development
Information.
Standard Comments:
29. A letter from Comcast is required to determine if any adjustments to their facilities or easements are
required.
30. Verify with Miami -Dade County that the proposed plat name is acceptable.
31. All encroachments across proposed tract and/or lot lines, must be removed prior to final plat
submittal.
32. An Opinion of Title, in the City of Miami Opinion of Title form, must be provided at the time of
fmal plat submittal.
33. Current backup documentation will be required for all who execute the fmal plat. A resolution for
authority to execute documents and a Certificate of Good Standing from the Secretary of State are
required, if applicable.
34. Tentative plat application must be made with Miami -Dade County after receiving approval from the
City of Miami Plat and Street Committee.
35. Be advised that an incomplete final plat package will not be accepted by the City of Miami. It is the
owner's responsibility to work with his/her surveyor and attorney to assure that everything is in
order before submitting the fmal plat package.
36. Be advised that if all requirements for scheduling the final plat for City Commission action are not
in order, the final plat will not be scheduled for a City Commission meeting.
37. A letter of concurrency from Miami -Dade County School Board is required prior to the final plat
submittal.
38. A letter from the Miami -Dade Water and Sewer Department is required to determine if any
adjustments to their facilities or easements are required.
39. A letter from Miami -Dade Water and Sewer Department is required to release the final plat in
connection with water and sewer service agreements.

40. Provide two 8 '/z" x 11" paper location sketch exhibits, one "Before Replat" and one "After Replat".
In addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements in the public rights -
of -way when property is platted. These subdivision improvements include paving, drainage,
landscaping, sidewalks, etc. In some cases, this could represent a substantial investment on your
part. REQUIRED: The submission of an electronic file of the tentative plat, in an AutoCAD, to the
City of Miami Resilience and Public Works Department, Development and Roadway Plans Section
will EXPEDITE the preparation and enhance the ACCURACY of the subdivision improvement
letter required for the final plat approval.
2. Execution of Covenant for Postponement of Subdivision Improvements is required in cases where
the required subdivision improvements are not completed.
3. The alteration, relocation or installation of utilities such as storm and sanitary sewers, electric,
telephone, water, etc., caused by this plat will be at the property owner's expense. Also, utility
easements may be required on the property being platted.
4. A building permit will not be issued on the property being platted until the final plat is recorded or
as authorized by the City of Miami Code Section 55-10(i). Also, the Temporary Certificate
Occupancy and/or Certificate of Occupancy for any building construction will be issued only after
all the required subdivision improvements have been completed.
5. Approval for fire flow requirements must be obtained from the Fire -Rescue Department prior to the
issuance of a building permit.
6. In order to mitigate problems associated with access and construction activities throughout the
neighborhood, the contractor/developer is encouraged to notify the affected neighborhood residents,
in writing, of the project starting and completion dates at the time of issuance of permits.
7. Additional items must be provided to the City of Miami Department of Resilience and Public
Works before the final plat is submitted to the City Commission for approval. You will be notified
in writing as to what these items are. It is required that the applicant contacts the City of Miami
Resilience and Public Works Survey Section to verify that all final plat submittal documents are
complete prior to final plat submittal.
8. Tentative plat approval is only valid for 1(one) year and 6 (six) months from the date of the Plat and
Street Committee meeting at which time it was approved.
