LHANC will provide a nutritious meal to older adults, 60 years of age or older. This meal will assist these older adults who may not
have money and access to a full nutritious meal, free of charge. By participating in this program, clients also have access to additional
meals offered to those clients who are at the highest risk for malnutrition through other funding sources.
IMPACT AND PERFORMANCE:
Describe the overall expected outcomes and performance measures for this project/program.
And, list the supporting documentation that will be submitted with the Close Out Report:
66% of the seniors who receive a meal will maintain or improve their nutrition status.
This is measured by comparing the client's nutrition score on their initial assessment
to the nutrition score on their annual reassessment. LHANC uses the form established by the
Florida Department of Elder Affairs. At the close of the program year, LHANC will
provide documentation of all clients who received meals, the total number of meals
served and billed, the payments to the caterer for the meals ordered, and any other
