This PERMIT authorizes the following (removal / planting) of tree(s), shrub(s) and/ or plant(s) on City of
Miami (the "City") right of way adjacent to property and is issued pursuant to Chapter 17 of the Code of the City of Miami.This
permit shall also constitute a covenant running with the land that touches, concerns, and runs with the land and binds the Owner
and his or her successors, heirs, assigns and grantees. The Covenantor/Owner/Permittee (hereinafter the "Permittee") agrees to
maintain the tree(s), shrub(s) and/or plants(s), should any tree(s), shrub(s) and/ or plant(s) not thrive or become damaged due to
any cause whatsoever, the Permittee shall replace the tree(s), shrub(s) and/ or plant(s) at no expense to the City.
Permission is given for within the right-of-way along of the subject property per
plans on file. Permittee is responsible for repairing any damage to the public right of way as a result of this work.
The following conditions shall be adhered to:
1. Permittee shall contact the Building Department/Environmental Resources Division at (305) 416-1100 or (305) 413-0750 to
schedule the final inspection. Approved work and inspection shall be completed prior to the permit expiration date for the BD
number referenced on this document.
2. Prior to any planting or excavation in the Public Right of Way, it is the Permittee's responsibility to notify Sunshine 811 at
1 (800) 432-4770 to determine if any of the Permittee's facilities could be damaged by removing or planting trees, shrubs and/
or plants pursuant to Chapter 556, Florida Statutes.
3. Maintain the Certificate of Liability Insurance in good standing.
4. No trees or plant items shall be planted in the twenty-five (25) foot corner radius. All shrubs shall be a minimum of eighteen
(18) inches in height when measured immediately after planting and not to exceed thirty (30) inches in height.
5. No trees shall be planted less than ten (10) feet from a driveway approach.
6. Trees will maintain ten (10) feet clearance from any other new or existing tree.
7. All planting material shall be Florida Grade #1 or better.
8. No loose materials that could slip out of place such as gravel, rocks, or mulch shall be placed in the right-of-way.
9. Tree replacement, if applicable, shall be the responsibility Permittee pursuant to Section 17-6 of the City Code, as amended.
10. The landowner is responsible for the watering, trimming, root pruning and all other maintenance of the planted trees, shrubs
and/ or plants for perpetuity.
11. For all non-standard improvements in the right-of-way, the Permittee must obtain a Non -Standard Improvement Covenant from
the Resilience and Public Works Department prior to the commencement of the work.
12. Permittee must obtain authorization from adjacent property owner(s) of planting locations prior to tree installations, if applicable.

Tree Permit Process and Checklist
Tree, Shrub and Plant Permit Process
This checklist is designed to list the supporting documentation needed for compliance with tree, shrub and plant
permit applications. The Checklists are to be used for the following projects:
➢ New construction with the removal, relocation or root pruning of trees (A)
➢ New construction with no tree impacts, tree removals, relocations or root pruning (B)
➢ Stand Alone Tree Permit for trees being removed, relocated or pruned that's unrelated to construction (C)
➢ Plantings and removals in City Right -of -Ways (D)
*A combination of checklists may be applicable depending on the scope of work
*Master Building Plans must include the Tree Permit plans as it will be processed as a sub permit.
Review:
1. Applications are reviewed for compliance in the order that they are received.
2. Corresponding comments are issued appropriately. Status of applications can be viewed using the City's
iBuild system at http://portal.miamigov.com/landmanagement
3. Once the application is approved, the Department will issue a notice of Intended Decision. This notice is
posted onto the subject property or adjacent to for ten (10) calendar days which provides all interested
parties the opportunity to review and appeal the Intended Decision.
Issuance of Permit:
4. If no appeals are received within the appeal period, the applicant is notified to pick up the application and
pay for the permit.
5. Once payment is made, the permit is issued accordingly.
Final Inspection:
6. The final inspection shall be completed no more than six (6) months from the approval date of the tree
permit. This inspection shall be approved prior to requesting a Temporary Certificate of Occupancy (TCO)
or Certificate of Occupancy (CO) from the Building Department.
7. To request the final inspection, the applicant shall call the number indicated on the permit card or the
Environmental Resources Division at 305.416.1551.
8. The inspection is conducted within 24-48 hours of the requested date.
9. Upon approval of the final inspection, the permit is approved and finalized.

o Tree/Boundary Survey* depicting all trees on -site, including those in the right of way.
For a commercial property, you must contact a Florida Licensed Surveyor to obtain a
Tree/Boundary Survey. The survey can not be more than one year old, and must include:
1. Tree Species
2. Location
3. Height
4. Spread and diameter
5. *NOTE: For a residential or duplex property, the survey can be prepared by the
owner or a owner's representative, as long as it reflects the above information.
o Certificate of Liability Insurance naming the City of Miami as an additional insured.
o Tree replacement plan/ Landscaping Plan drawn to scale including*:
1. New locations of relocated and newly installed trees.
2. Lot dimension.
3. Centerline(Show info)
4. Monument Line.
5. Demonstration that there is adequate spacing between trees, hardscapes and utility
lines.
o If the minimum tree replacement requirement cannot be met/planted on -site, applicant
must make alternative solutions to meet this requirement, including planting off -site or
Tree Trust Fund*.
o Photographs of the tree(s) and/or an Arborist Report(Show info)*.
