4. Received confirmation on 11/17/2021 of Miami Dade County DERM application for process
number MUA22001676. (Exhibit D) and (Exhibit E).
5. Receipt for payment of $80.62 for upfront fee DERM application dated 11/29/2021 (Exhibit
F). This completed the application process for Miami Dade County DERM.
STEP 3
1. Requested inspection from City of Miami Fire Prevention and received approval on
9/28/21 as reflected on (Exhibit G)
2. Requested inspection from City of Miami Code Compliance and received approval on
10/29/21 as reflected on Exhibit (G). This completed the City's inspections until such time
as we received DERM's approval. This is where the process seemed to slow down. During
this time due to COVID 19 the Miami Dade County DERM office was closed to the public
and they're employees and reviewers were working remotely. This caused the review
process which under normal circumstances takes several months to take an unusually long
time. And the lack of ability to contact the reviewers to lose this process. An important fact
to take into account is that during this process City and County are being done in separate
lanes. Which can lead to a lot of confusion.

1. Do not operate the business until a Certificate of Use, a Business Tax Receipt and. d applicable. a Certificate of Occupancy are issued.
2. This inspection tee is not refundable.

4. Pursuant to the Miami Charter Sec-2-207(b)' M8 applications for Certificate of Use shall expire 901'1(01de. days from the date Of application it approval is not

6. Schedule the reRimed inspections with the agencies selected on page two of the Inspection Forrn.
7. To schedule your inspections with the City of Miami Fire -Rescue and Code Compliance Departments. you must call 311 or 305-468-5900.
8. To request an inspection from the Florida Department of Business 6 Professional Regulation. Division of Floats 6 Restaurents, you must cat 850-487-1397 a

11. Once you have passed as the required inspections. please email this form and the DERM approval document as a PDF attachment directly to the City of
Miami Zoning Department.
