Miami -Dade County requires that the President/Chairman of the Board execute the Agreement on behalf of the agency. However, the Executive Director may execute the Agreement if approved by a resolution of the agency's Board. A copy of the applicable Board resolution(s) must be submitted with the Agreement. In addition, the agency must affix the corporate seal to the signature page of the Agreement or notarize it accordingly. Ellis 1) Welcome and Introductions Mr. Book opened the meeting at approximately 10:43 a.m., welcomed and proceeded to consider the meeting agenda. Manuel Sarria Victoria L. Shannon Williams, Esq. 2) Reasonable Opportunity for the Public to be Heard Mr. Book opened the meeting for Reasonable Opportunity for the Public to be Heard on any item on the agenda. Not seeing or hearing anyone come forward; Reasonable Opportunity was closed. 3) Approval of Minutes Mr. Book inquired if there were any comments, questions or changes to the December 2022 meeting minutes; not hearing any, Ms. Vidal made a motion to approve the minutes. Mr. Bell seconded the motion. All members present approved the motion set forth unanimously. 4) New Business a) Hotel -Motel Allocation (Stephen Alvarez recused himself from the meeting). Ms. Vidal made a motion to approve the item being set forth. Mr. Bell seconded the motion. Ms. Mallette referred members and guest to the memo provided in the meeting material. She explained the criteria for the uses of hotel placement. The impact of the pandemic related eviction moratorium significantly slowed in the inflow of families. Also, during this time Miami -Dade County relied on both the Coronavirus Aid, Relief and Economic Security (CARES) Act and American Rescue Plan Act (ARPA) to assist families experiencing homelessness. As in FY2019-2020, in FY2022-2023, the Trust budgeted $660,000 for hotel -motel (allocated to the City of Miami Homeless Assistance Program) and $94,500 for hotel -motel case management (allocated to Chapman Partnership). The end of the eviction moratorium and the current rental crisis has the shelters and hotels at capacity. From October 1, 2022 to January 17, 2023, 109 households, representing 442 individuals have been served in hotel -motels. Seventy-one percent ($468,000) of the current hotel -motel budget has been expended by the City of Miami. Additionally, the contracted hotel rates have increased to an average of $102.56 per night and are at capacity. As a result, the Trust has turned to New Hope CORPS and Hermanos de la Calle (Brothers on the Street) to assist in hoteling families, which are contracted for Street Outreach not hotels. Page 1 of 4 AGENDA ITEM 3 From October 1, 2022 to January 17, 2023 New Hope CORPS has incurred $30,703.69 (average night $77.24) in expenses with hotel placements; and Hermanos de la Calle has incurred $25,857.00 (average night $79.98) in expenses with hotel placements to assist households with minor children. At the request of the Trust, the City of Miami recently conducted market research as to whether existing contracted rates remain competitive and to identify additional vendors able to increase capacity. The City has recommended issuing an Invitation for Bid (IFB) to create a pool contract for additional hotel/motel rooms. All qualifying hotels -motels would be allowed in the pool and it would remain open so that additional hotels -motels can be added when identified. The total amount budgeted ($660,000 hotel and $94,500 for case management) is not enough to support in the influx of families through the end of the year. The Trust estimates that a total $1.9 million is needed to cover the anticipated hotel -motel expenses through September 30, 2023. There was discussion and questions among board members; Mr Arteaga inquired as the enrollment of the children in school and the provision of needed services. Mr. Book requested the information be forwarded to Mr. Arteaga. Staff recommends that an additional $1,145,000 be allocated for hotel -motel from Uncommitted Carryover to support the continued placement of homeless families through September 30, 2023 and reimburse both providers above for expenses already incurred and continue to use them as resource if necessary. All members present approved the motion for the reimbursement of New Hope CORPS and Hermanos de la Calle as presented and outlined in the memo including any corrections for any deficiencies to make the providers whole. All members present approved the motion set forth unanimously. (Mr. Alvarez re -joined the meeting and William Porro recused himself). Ms. Fernandez made a motion to approve staff recommendation to allocate $1,145,000 from Uncommitted Carryover for hotel -motel placements of homeless families through September 30, 2023, allowing Trust staff discretion to determine how funds will be dispersed. Ms. Vidal seconded the motion. All members present approved the motion set forth unanimously. b) U.S. HUD System Performance Analysis and Assessment Ms. Mallette referred members and guests to the memo provided in the meeting packet and provided background as to the grant agreement with ASHLIN Management Group for a U.S. HUD System Performance Measure automation discovery and assessments. The objective was to identify current processes, procedures and usage of data, analysis of data sets and flows, and system users work with data. During Phase I, staff gained a better understanding of limitations and needs, and during this same time, data visualization software company Tableau began offering a smart table product that saves visualization logic and allows Trust staff to build and design in-house dashboards using actual U.S. HUD data logic for a fraction of the cost. In past years, building a dashboard was time consuming because the HMIS Management team would have to export the data and design the visualization tables for each of the measures, including filters and sub -populations making it highly time consuming and without the preferred frequency. Staff proposes using funding already budgeted for data enhancement and add a Business Analyst position to the Homeless Trust staff rather than enter into Phase II with a third party. This individual will be dedicated to learning from high performing providers and evaluate poor performer's policies and practices in an effort to isolate changes that will improve performance. Staff recommends HMIS staff work with stakeholders to continue building System Performance Measure dashboards using the Tableau visualization tools at a an annual cost of approximately $840. Secondly, to add a Business Analyst (annual salary range is $64,413 — 109,175) to the Homeless Trust staff to work with the HMIS team and contracted providers on total quality improvement as it relates to the U.S. Page 2 of 4 AGENDA ITEM 3 HUD System Performance Measures. Both the software tool and the position will come from an existing line item in the FY2022-2023 budget for technology advancements. Ms. Vidal made a motion to approve staff recommendations as presented and outlined in the memo. MS. Caravia seconded the motion. All members present approved the motion set forth unanimously. c) Cold Weather Reports: 12/22 to 12/27/2022 and 1/13 to 1/15/202 Ms. Mallette referred members and guests to the reports for the two (2) cold weather events during the winter months. One off -site location, and ISD facility, was staffed by New Hope CORPS with Chapman Partnership and Miami Rescue Mission supplying meals and supplies. As always every effort is made to engage and retain individuals post emergency as this is an opportunity to connect people with housing and services. In December 44 of 183 people were retained (24%); and in January 59 of 191 were retained (31%). 5) OLD BUSINESS a) Emergency Housing Voucher Report Ms. Mallette referred members and guests to the presentation provided in the meeting packet and provided background and unit status. We are having weekly meetings with the housing authorities and making good progress. We had 770 vouchers, we are searching for units for 57 households, 63 units identified and 724 have been housed. Our lease -up percentage continues to rise: a 85% lease -ups in Miami -Dade, Hialeah is at 86% lease -up, Miami Beach is at 58%, and Homestead is at 65% lease -up. This is a one-time voucher with the term to lease -up expiring December of 2023. This is an information item only. 6) REPORTS a) The MNetwork Report Lisa Mozloom provided an overview of public relations activities that mirrors the report provided in the meeting packet with media clips, including but not limited to, the following: Steady flow of consistent media coverage from end of December to January; top performing matter was cold weather outreach • Blog Post: "An Actual Home for the Holiday" is in the packet Extended Poster and Essay Contest until January 27, 2023; still receiving applications; $300 cash prize for student and teacher Homeless Awareness Day: 12t" Anniversary: video of history of Homeless Awareness Day played Mr. Book made several comments on the work of the M Network. b) Apple Tree Perspectives Report The report provided in the meeting packet for review. Any comments or questions may be directed to the Homeless Trust Office. Ms. Caravia made a motion to accept the report. Ms. Fernandez seconded the motion. All members present approved the motion set forth unanimously. c) Chapman Partnership Report Ms. Sarduy provided updates on the operations and activities at the centers, including but not limited to, the following: Thirty-one cold weather placements Client capacity is at 100% at both centers; average length of stay is 131 days at both centers • 55% Outplacement (without neutrals) to permanent housing pathways, which is lower, reflective of the current housing market; • With assistance of the Homeless Trust providing pathways to permanent housing: seven (7) individuals and nine (9) families in December 2022; 51 individuals YTD; total of 16 households • Employment: 81 job placements YTD with an average hourly wage of $15.4 • Chapman tour by U.S. HUD Secretary Marcia Fudge recognizing our community and encouraging other communities to replicate ours; HUD representative will be at our PIT Count this year Page 3 of 4 AGENDA ITEM 3 ♦ Nextgen Spelling Bee on January 25th, 2023; 15 volunteers will be at North Center to assist students; this is to encourage and motivate our students d) Economic Indicator Report Referring to the memo and attachments in the meeting packet, Ms. Mallette stated that we experienced a 8.3% increase in Food & Beverage Tax revenue in December 2022 as compared to the previous year, representing $2,767,674.83, and year-to-date collection is $8,320,653.62. The national unemployment decreased in November to 3.6% from the previous month, the State decreased to 2.6%; and Miami -Dade County also decreased to 1.8% from the previous month. According to U.S. Department of Labor Statistics, the total number of people unemployed in Miami -Dade County in September was 23,753. The Homeless Helpline received a total of 25,869 from July to October; detailed information is provided in the utilization report in the memo attachment. e) Executive Director's Report Ms. Mallette provided updates on Homeless Trust operations and activities as follows: • The Point -in -Time Count will be conducted on January 26, 2023. U.S. HUD representatives will be participating in this event; Board members are invited and encouraged to participate • February 7, 2023 is the last Racial Equity Partners training 7) OTHER ITEMS With no further business to be discussed, the meeting was adjourned at approximately 11:21 a.m. Ronald L. Book, Chairman Page 4 of 4 ATTACHMENT A, SCOPE OF SERVICES EMERGENCY HOTEL/MOTEL PLACEMENT PROGRAM PC-2021-HTMT-1 SCOPE OF SERVICES EMERGENCY HOTEL/MOTEL PLACEMENT PROGRAM The Provider agrees to provide emergency hotel/motel placement of homeless families with children for a period of up to seven (7) days in area hotels/motels. Thereafter, the provider must obtain the approval of the Miami -Dade County Homeless trust for additional days as needed on a case -by -case basis. Families may be provided food vouchers on an as -needed basis of up to $20.00 per diem while residing in hotels/motels. Families with more than four (4) members may be provided an additional $5.00 per person per day. Reimbursement will only be made for properly documented disbursement of food vouchers. All reimbursements must be submitted to the County by the 10t" day of each month following the month of service. All reimbursement requests must be approved by the County prior to the disbursement of funds. Scope A-2 Scope of COVID-19 Expenditures The County is instructing Provider to undertake protective measures to prevent or mitigate the spread of COVID- 19 during the period in which public officials advise COVID-19 special measures should be taken. The County will reimburse Provider for expenses incurred in taking such protective measures during such time period. Allowable COVID-19 expenditures are set forth below. The County has sole discretion to determine if expenditures were made for the purpose of preventing or mitigating the spread of COVID-19 and the dates of the period in which public officials advise that COVID-19 special measures should be taken. Total reimbursement for incurred COVID-19 costs under this Agreement shall not exceed $ N/A without the County's prior written approval. Allowable COVID-19 Expenditures: • Personal Protection Equipment (PPE). • Testing and screening for COVID-19. • Cleaning supplies and/or cleaning services by outside vendors, including application of an antimicrobial surface protectant. • Physical modifications specifically undertaken to prevent or mitigate COVID-19 spread within the facility. • Ventilation -related supplies or modifications, except for substantial modification or replacement, and installation and maintenance of UV lighting. • Educational material and signage specific to COVID-19. • Additional food costs incurred for children in residence attending school remotely. • Staff overtime incurred due to staff absences resulting from their COVID-19 infection, quarantine after exposure to another person tested positive for COVID-19 or care of a household member with COVID- 19 infection. Provider may request reimbursement for the cost of temporary staffing necessary to cover the absent permanent employee's hours that is above and beyond the budgeted cost of the absent permanent employee. • Other COVID-19 justified expenditures. Payment Processes and Documentation Requirements: Payment Processes: Provider must submit a monthly invoice with back-up documentation attached, comprised of Provider's account ledger for COVID-19 expenditures, invoices and receipts which include proof of invoice payment, canceled checks, time sheets and payroll registers and other documentation as requested. The County will pay Provider within thirty (30) days of the County's receipt of the invoice. Additional Documentation Requirements: • Provider must establish a cost center or the equivalent specifically for COVID-19 expenditures. The account ledger for the cost center or the equivalent must be submitted with Provider's invoice. Such ledger must list: (a) the purchased item or service, (b) vendor name; (c) vendor's invoice number with purchase order date or receipt with same; (d) payment amount; (e) payment date; (f) check number unless paid online. • Vendor invoices, purchase orders or receipts must have a notation that they are COVID-19 expenditures. • Request for reimbursement of costs incurred due to a permanent employee's absence must be supported by Human Resource documentation of the basis for the employee's absence and period of absence with the employee's name and any other identifying information redacted. Time sheets and payroll records documenting an employee's overtime must be included in the reimbursement request and indicate that such overtime was necessary to cover the absent permanent employee's hours. Invoices for temporary staffing must match the period in which the permanent employee was absent and indicate that such staffing was necessary to over the hours of the absent permanent employee as well as include Human Resource record of permanent employee's budgeted salary. • For any expense not expressly described above, Provider must provide a narrative justification that the expense was incurred in response to COVID-19. The policy outlined the types of critical incidents which must be reported to the Continuum of Care's Incident Report Coordinator, Miguel Pimentel. For each critical incident, a report must be submitted to the Miami -Dade County Homeless Trust within one business day. When a critical incident occurs, subcontracted provider staff should 1) take action to ensure the health, safety and welfare of all individuals involved in the incident, and 2) contact law enforcement, emergency responders of the Abuse Hotline. The incident reporting form has been significantly updated to include both wrong -doing, as well as allegations of wrongdoing. Altercation. A physical confrontation occurring between a client and employee or two or more clients at the time services are being rendered, or when a client is in the physical custody of the department, which results in one or more clients or employees receiving medical treatment by a licensed health care professional. b. Client Death. A person whose life terminates due to or allegedly due to an accident, act of abuse, neglect or other incident occurring while in the presence of an employee, in Homeless Trust contracted program facility. c. Client Injury or Illness. A medical condition of a client requiring medical treatment by a licensed health care professional sustained or allegedly sustained due to an accident, act of abuse, neglect or other incident occurring while in the presence of an employee, in a Homeless Trust contracted program. d. Other Incident. An unusual occurrence or circumstance initiated by something other than natural causes or out of the ordinary such as a tornado, kidnapping, riot, or hostage situation, which jeopardizes the health, safety and welfare of clients. e. Sexual Battery. Any allegation of a program participant or program staff intentionally touching a minor or another person without their consent. This includes incidents of inappropriate verbal offenses, incidents that occur outside of the residence, and incidents were the program participant was victimized by someone outside of the residence. Incidents involving a minor, person who is 60 or older, or someone who is disabled must be reported to the DCF. f. Abuse or Neglect. Any physical maltreatment of a child, disabled person, or someone age 60 or older. Any failure to act on the part of the parent or care taker, which results in harm to a child, disabled person, or someone age 60 or older. g. Suicide Attempt. An act which clearly reflects the physical attempt by a client to cause his or her own death while in the physical custody of the depail.inent or a departmental contracted or certified provider, which results in bodily injury requiring medical treatment by a licensed health care professional. h.
