The Program, if fully supported by State and County resources, will reach an administration goal of over 45,000 vaccines per week. The Program is based on the guidelines and recommendations set by the Centers for Disease Control and Prevention (CDC), the Florida Department of Health (FDOH), and the Florida Division of Emergency Management (FDEM). The Program will not be successful without collaboration and partnership from both County and State Emergency Management agencies, the FDOH, along with assistance from private organizations. Additionally, support from the Florida National Guard as well as the federal military may be required to fully execute this plan. All identified resources will be necessary to meet the weekly vaccine goal and to reach the most vulnerable and underserved areas of our community. Problem Statement According to the CDC, immunization with a safe and effective COVID-19 vaccine is a critical component of the United States strategy to reduce COVID-19 related illnesses, hospitalizations, and deaths'. Currently, operations at vaccination sites throughout the country are limited by vaccine availability. This plan operates under the assumption that a sufficient number of vaccination doses will be allocated to the City of Miami on a consistent basis to ensure continuity of operations. As compared to other local areas, the City of Miami demonstrates a larger than average sector of the population that is 65 years and older. According to the U.S. Census Bureau (Figure 1), the 65 year and older population in the City of Miami accounts for 16.9% of the population, or approximately 80,000 residents. Additionally, the U.S. Census Bureau reports that 23.4% of residents within City of Miami live under the poverty line, and 23.2% percent of its residents under age 65 are without health insurance3; both segments may be disproportionately vulnerable to COVID-19 due to a lack of access to health care. These factions consist of the following: • Major Points of Distribution (drive-thru vaccination sites) targeting any and all residents that are able to participate by vehicle. • Walk-up Points of Distribution (PODs). • Mobile Vaccination Units targeting the severely disabled and homebound. • Partnership with FDEM & CDR Maguire targeting Assisted Living Facilities (ALF s) • Partnership with FDEM & CVS/Walgreens targeting nursing homes and group homes • Partnership with Miami -Dade County Public Schools within City of Miami boundaries targeting students, as well as faculty/staff City of Miami Major Points of Distribution (Major PODs) Miami Marine Stadium A drive-thru vaccination site is proposed for the Miami Marine Stadium located at 3501 Rickenbacker Causeway, Miami, FL 33149. This drive-thru vaccination location is for City of Miami residents and will be conducted by appointment only. Proposed vaccination hours are 0800-1600 hours. Operational Overview - Vehicles will enter the site from the Rickenbacker Causeway through the main entrance of Marine Stadium. The traffic control pattern leading into Marine Stadium will be designed in partnership with law enforcement. Two vehicular lanes will enter Marine Stadium leading to an Entry Control Point (ECP) staffed by security personnel. This drive-thru vaccination location is for City of Miami residents and will be conducted by appointment only. Proposed vaccination hours are 0800-1600 hours. Operational Overview - Vehicles will only be able to enter Miami Dade Auditorium while traveling westbound on West Flagler Street. Once they arrive at the auditorium, vehicles will make a right and proceed to the two lanes located on the east side of the property. Vehicles will travel towards the rear of the auditorium building where they will reach the Entry Control Point (ECP). Checkpoint one will confirm the appointment and instruct residents to proceed to one of six vaccination lanes (Figure 5). If a resident does not have an appointment, they will be advised on how to make an appointment and continue to the access road to exit property. Once in a vaccination lane, citizens will be registered, vaccinated by nurses, and instructed to proceed to one of four observation lanes. While under observation, recipients will be monitored for 15-30 minutes (depending on past medical history) for signs and symptoms of an allergic reaction. Citizens will then be released and shall enter exit lanes located on the west side of the auditorium. The site will accommodate current testing lanes as well as new vaccination lanes. Vaccinations will be provided for Miami -Dade County residents who meet the FDOH vaccination criteria. Residents can schedule an appointment by utilizing Miami -Dade County's 311 service or through FDEM's online platform. The Marlins Park vaccination and testing site is a collaboration between City of Miami, Miami -Dade County, FDEM, and the Florida National Guard. Testing commenced March 25, 2020 and hours of operation are 0800-1500 hours. A maximum of 800 appointments a day are scheduled through Miami -Dade County's 311 division and/or online platform. Operational Overview - Vehicles will enter on the east side of the property at NW 7th St and 14th Ave where they will follow the cones to 4th St and 16th Ave for appointment verification. Vehicles with appointments will make a right on 16th Ave and proceed to Lot 1 tents where the registration process will begin. Vehicles without proper appointment confirmation will be instructed on how to make an appointment and directed to continue straight on 4th St to exit the property. After the registration in Lot 1 vehicles will proceed to Lot 2 for vaccination. Post vaccination, vehicles will proceed to Lot 3 where they will be monitored for 15-30 minutes (based on past medical history) by a health care provider for possible allergic reactions. The walk-up vaccination location is for City of Miami residents and hours of operation will be 0800-1600 hours. Operational Overview - Citizens with an appointment will enter the vaccination site through the parking lot entrance located between 1st St and 2nd St on 9th Ave. The vaccine line will form in front of the registration tent (east of parking lot) where a minimum of two nurses will be ready to confirm appointments and complete the vaccination card before directing to the vaccination line. Once vaccinated, residents will be directed to the observation tent where chairs will be provided for the 15-30-minute observation period (depending in medical history). The target audience for the MVP will be homebound residents aged 65 and older. The MVP has dual components, the Mobile —POD (M-POD) and the Mobile Vaccination Unit (MVU). The M-POD can administer 60-80 vaccines per operational period in one location; with the advantage that this mobile point of distribution can be relocated throughout the City. Sites will be identified where needs are greatest as observed in public housing and senior -only resident buildings. Faith Based Organizations (FBOs) in each of the five districts of the City of Miami may serve as a link between communities, local health, and other officials. FBOs can provide insight about the feasibility and acceptability of proposed mitigation strategies and informing authorities about resources and needs in their communities. Depending on the nature of their work, an FBO's stakeholders may include volunteers, members, clients, supporters, patrons, program participants, and event attendees. Partnerships with FBOs reveal promise to serve as host for the M-POD in ethnically diverse communities. The M-POD can operate seven (7) days per week for eight (8) hours per day. 26 Submitted into the public record for item(s) NA.2, on 01-14-2021, City Clerk M-POD Operational Overview • (1) "Incident Commander" to oversee the daily operation of the MVP • (1) City of Miami Police Officer to oversee security throughout the operation • (1) "Documentation Manager" to assist with any documentation and paperwork • (1) "Dispatcher" to coordinate response and provide alarm numbers. • (1) "Registration Screener" to conduct Covid-19 health screenings and ensure all vaccine recipients sign in and out. • (1) "Vaccination Manager" to secure pickup, preparation, and return of vaccines with Fire Logistics. • (1) "Vaccine Transporter" to assist with pick-up and delivery of supplies. • (1) Paramedic and/or Nurse to administer per protocol established by the vaccination company and standing orders from MFR Medical Director. • (1) "Observation Manager" to manage the flow of individuals pre- and post - vaccination. Mobile Vaccination Unit (MVU) For the most severely disabled who are unable to mobilize out of their apartment or home, a mobile vaccination unit (MVU) can be implemented. The MVU provides vaccinations within a resident's home, the MVU is a lower volume unit limited to approximately 10 vaccines per day for those residents identified as non -mobile. MVU Operational Overview - With the approval of the department's medical director, a minimum of two (2) certified paramedics and one (1) emergency medical technician would be needed to deliver the vaccinations. Each member will fulfill a defined roll to meet: command 27 Submitted into the public record for item(s) NA.2, on 01-14-2021, City Clerk and control, quality assurance, application of protocols, documentation, proper handling, and administration of the vaccination. The staffing would also ensure that adequate personnel is on hand to immediately treat any life -threatening emergency caused by an adverse reaction to the vaccine. Safety All personnel will be appropriately trained by the department's Occupational Health Nurse on the administration of the vaccination. Personnel shall be furnished with the appropriate level of personnel protective equipment (face shields, respiratory protection, gowns, gloves, etc.) to perform the task of administering the vaccinations. All recipients of the vaccine will be closely monitored for a minimum of fifteen (15) minutes after receiving the vaccination and according to manufacturer recommendations. Individuals with known allergies to vaccinations will be observed for thirty (30) minutes. The inefficiency of a mobile vaccination unit (MVU) demands an effective transportation plan to vaccination points -of -distribution for the elderly and those without means of transportation. Transportation plan may include private partners, public transportation, or city -managed transportation services. The ideal transportation service will transport those in need to existing walk-up PODs. Miami -Dade County Public Schools There are currently over 80 primary and secondary schools within the City of Miami that are comprised of both public, private, and charter schools (Appendix 1.) They account for over 20% of all schools in Miami -Dade County with an estimated student population of over 70,000 students. As the CDC releases recommendations for the vaccination of minors, efforts should be made with Miami Dade County Public Schools (MDCPS) to ensure that all children in the City of Miami have availability and opportunity to receive the COVID-19 vaccine. The City of Miami proposes coordination between Miami -Dade County Emergency Management, and MDCPS for the development and implementation of a vaccination program for students in the local surrounding community. Vaccination Strike Teams (VSTs) from FDOH and FDEM may also be valuable partners in accomplishing this endeavor. They are a Nixle Keyword and a Member Portal. Both methods can be used for targeted vaccine information including reminder information for individuals who are scheduled for a second dose of vaccine. Nixle Keyword: A keyword will be created, i.e.- "MIAMIVACCINE" and have residents opt -in by texting the keyword to 888-777. The system can then be used to send alerts when vaccination appointments are open, general reminders about second doses, survey links for side effects, etc. Nixle is also an anonymous system which can be used to relay general information about an event. Member Portal: For targeted messaging, we can create additional information fields within the application for people who have received the vaccination so they may sign up for targeted reminders for their specific second dose vaccination date.
