A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
BY A FOUR -FIFTHS (4/5THS) AFFIRMATIVE VOTE, AFTER A DULY NOTICED
PUBLIC HEARING: (1) WAIVING COMPETITIVE BIDDING PURSUANT TO
SECTION 18-86 OF THE CODE OF THE CITY OF MIAMI, AS AMENDED, (2)
AUTHORIZING THE CITY MANAGER TO ACCEPT A GRANT AWARD IN A
TOTAL AMOUNT NOT TO EXCEED $25,000.00 ("GRANT") FROM THE
SIMPLY HEALTHCARE COMPANY ("GRANTOR") FOR THE CITY OF MIAMI
("CITY") LIVE HEALTHY LITTLE HAVANA INITIATIVE ("PROGRAM") — MIAMI
DIABETES PROJECT ("PROJECT") TO COMPLEMENT AND ENHANCE THE
PROGRAM FOR THE PROGRAM YEAR FROM JANUARY 1, 2018 THROUGH
JANUARY 31, 2019, AND (3) AND AUTHORIZING THE CITY'S IN -KIND
CONTRIBUTION OF PUBLICITY AND A SOCIAL MEDIA TOOL KIT IN AN
AMOUNT NOT TO EXCEED FIVE HUNDRED DOLLARS ($500.00) ("CITY IN -
KIND CONTRIBUTION"); ESTABLISHING A NEW SPECIAL REVENUE
PROJECT FUND ENTITLED "LIVE HEALTHY LITTLE HAVANA INITIATIVE —
MIAMI DIABETES PROJECT" AND APPROPRIATING THE GRANT AND THE
CITY'S IN -KIND CONTRIBUTION, SUBJECT TO BUDGETARY APPROVAL AT
TIME OF NEED, IN ORDER TO FUND THE PROJECT; FURTHER
AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE, IN A
FORM ACCEPTABLE TO THE CITY ATTORNEY, A GRANT AGREEMENT
AND THE OTHER NECESSARY DOCUMENTS, AMENDMENTS,
EXTENSIONS, RENEWALS, AND MODIFICATIONS IN ORDER TO
IMPLEMENT THE ACCEPTANCE OF AND COMPLIANCE WITH SAID GRANT
AWARD FROM THE GRANTOR FOR THE PROJECT.
WHEREAS, in February 2014, the Health Foundation of South Florida ("Foundation")
launched an initiative aimed at strengthening community capacity to plan collaboratively and
collectively carryout strategies to improve health; and
WHEREAS, after an extensive selection process, the Foundation selected the City of
Miami ("City") neighborhood of Little Havana to invest up to $3.75 million over a six (6) year
period; and
WHEREAS, "root causes" of health issues in Little Havana have been identified based
on an assessment of available data and input from community residents, which resulted in a
comprehensive Community Action Plan aimed at addressing those root causes that have been
prioritized; and
WHEREAS, since 2015, the City has been responsible for the administration and
coordination of the Live Healthy Little Havana Initiative ("Program") to ensure broad, multi -
agency, and resident representation in the initiative; and

WHEREAS, the Simply Healthcare Company ("Grantor") has awarded the City Twenty -
Five Thousand Dollars ($25,000.00) ("Grant') for the Live Healthy Little Havana Initiative —
Miami Diabetes Project ("Project") to work in conjunction with Miami -Dade Area Health
Education Center ("AHEC's") Diabetes Empowerment Education Program ("DEEP") for
physical activity programming and cooking demonstrations for the purpose of reducing Type 2
Diabetes in Little Havana; and
WHEREAS, the funds will be utilized to provide physical activity programs,
nutritional/cooking demonstrations, and information on other health related topics to 200
participants engaged in the AHEC and DEEP program in the Little Havana neighborhood; and
WHEREAS, the City will further assist the Project by providing publicity and a social
media tool kit in an amount not to exceed Five Hundred Dollars ($500.00) ("City In -Kind
Contribution") which is subject to budgetary approval at time of need; and
WHEREAS, the City wishes (1) to accept the Grant from the Grantor for the Project and
(2) to establish a new Special Revenue Project Fund and to appropriate the Grant and the City
In -Kind Contribution, subject to budgetary approval at time of need, from a new Special
Revenue Project Fund in order to fund the Project in the total not to exceed amount of Twenty -
Five Thousand Five Hundred Dollars ($25,500.00); and
WHEREAS, in order to accept the Grant and to comply with the Grant's requirements
by providing the City's In -Kind Contribution of publicity and a social media tool kit, it is in the
best interests of the City to waive competitive bidding pursuant to Section 18-86 of the Code of
the City of Miami, as amended ("City Code"), by a four -fifths (4/5ths) affirmative vote, after a
duly noticed public hearing;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the Preamble to this Resolution are
adopted by reference and incorporated as if fully set forth in this Section.
Section 2. By a four -fifths (4/5ths) affirmative vote, after a duly noticed public hearing:
(1) the competitive bidding requirements of Section 18-86 of the City Code are waived; (2) the
City Manager is authorized' to accept the Grant from the Grantor to complement and enhance
the Program for the Program year from January 1, 2018 through January 31, 2019, and (3) the
City's In -Kind Contribution is authorized.
Section 3. The following new Special Revenues Project Fund is established and
resources are appropriated as described herein:

the City Attorney, including but not limited to, those prescribed by applicable City Charter and City Code
provisions.

Section 4. The City Manager is further authorized' to negotiate and execute, in a form
acceptable to the City Attorney, a grant agreement and the other necessary documents,
amendments, extensions, renewals, and modifications in order to implement the acceptance of
said Grant award from the Grantor for the Project.
Section 5. This Resolution shall become effective immediately upon its adoption and
signature of the Mayor. 3

3 If the Mayor does not sign this Resolution, it shall become effective at the end of ten (10) calendar days
from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
