Project Title
Healthy little Havana - Miami Diabetes Initiative
Please describe the health -related purpose of this grant. (150 words)
The City of Miami and Live Healthy Little Havana will partner with Miami -Dade AHEC to implement and
enhance the DEEP program, designed to empower and educate diabetes patients to take control of their
health by eating healthy and increase physical activity. LHLH will augment this program at all sites by
offering physical activity programming and cooking demonstrations. The proposed project will leverage
important community partners to empower and educate individuals living with chronic diseases to carry
out behavior that promote a healthy lifestyle. Following a collective impact approach with over 120
partners and 60 organizations, LHLH will leverage resources to ensure this program enhances the current
services offered throughout the community that allow active adults to socialize, get fit, and get educated
on health topics.

15 sites, each site will offer a 6 week program;
Interactive workshops (physical activity programs, nutritional/cooking demonstration
Various health topics (cooking, dance, yoga)
