ENCLOSURES:
Procurement Department issued a Request for Proposal (RFP) No. 681390 on May 12, 2017, to
procure a Risk Management Information System for the Department of Risk Management. During the
process of establishing an Evaluation Committee, we exhausted all resources in acquiring a third
external participant, with the required expertise. We replaced that person with an internal City of Miami
employee.
It is respectfully requested that you approve the following individuals as members of the Evaluation
Committee to evaluate and score the proposals. Each has the requisite knowledge and experience
to evaluate the proposals pursuant to the RFP.

Risk Management, City of Miami (COM).
