ALFONSO ("ALFONSO") AS CITY MANAGER OF THE CITY OF MIAMI ("CITY") EFFECTIVE JANUARY 11, 2018; FURTHER AUTHORIZING THE DIRECTOR OF FINANCE TO PAY ALFONSO THE EQUIVALENT OF SIX (6) WEEKS' PAY AND ALL EARNED BUT UNUSED VACATION AND SICK LEAVE AS OF JANUARY 11, 2018 UPON EXECUTING A SEPARATION AGREEMENT AND GENERAL RELEASE ("AGREEMENT"), IN A FORM ACCEPTABLE TO THE CITY ATTORNEY; AUTHORIZING THE MAYOR TO DESIGNATE FERNANDO CASAMAYOR, THE CHIEF FINANCIAL OFFICER/ASSISTANT CITY MANAGER, AS THE INDIVIDUAL TO EXECUTE SAID AGREEMENT ON BEHALF OF THE CITY; ALLOCATING FUNDS FROM ACCOUNT NO. 00001.980000.515000. WHEREAS, Daniel J. Alfonso ("Alfonso") was initially hired as the Director of the Office of Management and Budget of the City of Miami ("City") on or about August 8, 2011; and WHEREAS, on or about July 5, 2013, Alfonso was promoted to Chief Financial Officer/Assistant City Manager; and WHEREAS, on or about March 4, 2014, Alfonso was appointed as City Manager; and WHEREAS, Alfonso has served in all capacities, especially in the role of City Manager, with distinction and honor; and WHEREAS, Alfonso has indicated that he will be resigning as City Manager effective January 11, 2018; and WHEREAS, in recognition of Alfonso's dedicated and invaluable service to the City since 2011, the Miami City Commission wishes to pay Alfonso six (6) weeks' pay and all earned but unused vacation and sick leave as of January 11, 2018; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. Alfonso's resignation as City Manager effective January 11, 2018 is accepted. Section 3. The Director of Finance is authorized to pay Alfonso the equivalent of six (6) week's pay and all earned but unused vacation and sick leave as of January 11, 2018 upon executing a Separation Agreement and General Release ("Agreement"), in a form acceptable to the City Attorney. Section 4. The Mayor is authorized' to designate Fernando Casamayor, the Chief Financial Officer/Assistant City Manager, as the individual to execute said Agreement on behalf of the City. Section 5. Funds are to be allocated from Account No. 00001.980000.515000. Section 6. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.2 APPROVED AS TO FORM AND CORRECTNESS: ndez, City Attor ey 12/4/2017 1 The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to, those prescribed by applicable City Charter and City Code provisions. 2 If the Mayor does not sign this Resolution, it shall become effective at the end of ten (10) calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
