Procurement Department ENCLOSURES:
Procurement Department (Procurement) on behalf of Risk Department (Risk), will issue a
Request for Proposals (RFP) No. 680381 on March 30, 2017, for an Employee Group Disability
Insurance Program.
It is respectfully requested that you approve the following individuals as members of the
Evaluation Committee to evaluate and score the proposals. Procurement has made attempts to
locate a third external voting member, but to no avail. Each has the requisite knowledge and
experience to evaluate the proposals pursuant to the RFP.
