Your organization has been conditionally awarded a grant of recycling as listed in the
notification email you received. To finalize the grant, please complete the following
agreement indicating you agree to the terms of the grant and commits to fulfilling all of
the requirements. Note, if your notification message indicated the grant was also
conditioned on making an independent matching purchase of additional recycling bins,
you'll also need to confirm the purchase arrangement with the bin vendor to finalize the
grant award. All recipients will receive a separate confirmation email once their grant
has been finalized that also includes bin delivery details.

Grant recipients are required to submit a brief web based report on two occasions,
approximately six months after receiving the bins and again one year after receiving the
bins. KAB will send a reminder notice on each occasion with a link to the online report
form.
By accepting a grant of recycling bins, the grantee understands and agrees to the
following:
15.During the first year of use, granted recycling bins will be used for the purpose stated in
the application. Where substantial changes are warranted, the grantee must notify and
receive authorization from KAB in advance.

receipt of the grant and/or the bins. The grantee also agrees to send a copy of the release to
the designated KAB and Coca-Cola contacts listed in the notification email.

granted bins during the initial six months and within 1 year after receiving the bins. Where
actual weights are not available, the grantee is nonetheless expected to develop a system to

Response to Question 20.
Waste collectors and Parks maintenance staff will service all recycling bins and place recovered
materials for collection by a waste hauler. Data for recyclables collected from parks and
pedestrian walkways throughout the City will be directly reported to the Department of Solid
Waste. The City of Miami Department of Solid Waste franchisee agreement requires that waste
haulers report recycling data on a quarterly basis. Data collected will be analyzed by the
Department of Solid Waste and will be used to improve outreach efforts.
