The Florida Department of Transportation (FDOT) State Safety Office is pleased to inform you that your agency has
been selected to receive subgrant funding for the 2017 Federal fiscal year, which begins October 1, 2016. The
following concept paper has been accepted with the funding levels as stated below:

Based upon funding levels awarded to the State of Florida for the Federal fiscal year by the National Highway
Traffic Safety Administration (NHTSA), grant awards may not be the same as the amount requested in your concept
paper and the award does not mean that everything listed in the concept paper was approved and/or funded.
Attached is the new FDOT subgrant form that will need to be completed for execution of your subgrant funding.
The total amount of the subgrant cannot exceed the funding awarded in the Highway Safety Plan (HSP) as listed
above. Your assigned traffic safety program manager for this project is Kira Houge and you will need to coordinate
with them to work on the details of your approval, complete the subgrant form, and submit for processing.
There also have been significant changes to State and Federal requirements within the Part V boilerplate language for
all subgrants in FY2017. ***Please review the attached memo regarding substantial changes.***
All FY 2017 subgrants MUST be submitted using the new FDOT Subgrant form attached hereto. An electronic
version can also be obtained by contacting your traffic safety program manager. Subgrants submitted on old forms
will be returned for resubmittal on the new form.

Subject: Substantial Changes to FY2O17 Date: September 9, 2016
subgrant requirements and limitations

There have been several changes in State and Federal laws which required significant changes to the
way traffic safety subgrant funds can be used and the reporting requirements. The following highlights
a majority of the changes that have been made to all subgrants awarded for FY2017.
Part IV: Quarterly Progress Reports and Performance Indicators Changes:
• The quarterly progress report has been replaced with performance reports that must be
submitted with EACH request for reimbursement.
• Milestones have been replaced with the minimum performance standards and NHTSA
required activity reporting in the performance report.
Part V: Acceptance and Agreement Changes:
• Number 1. Reports - The verbiage in Part V has been modified to replace the requirement of
quarterly progress reports with the new requirement to submit performance reports with
EACH request for reimbursement. And the Final Narrative Report (FDOT Form No. 500-065-
20), has been updated to include NHTSA Required Activity Reporting.
• Number 4. Approval of Consultant and Contractual Service Agreements The verbiage in
Part V has been modified to clearly express that "All consultant and contractual service
agreements (CSA) must be submitted to the FDOT Safety Office in draft form for review and
approval'. The requirement for vendor certification and statement and signature on CSA
invoices has also been removed.
• Number 6. Travel - The verbiage in Part V has been modified to clearly express that the FDOT
State Safety Office shall not pay for overnight lodging/hotel room rates that exceed $150.00
per night (before taxes and fees).
• Number 16. Procedures for Reimbursement - All requests for reimbursement MUST include a
performance report (FDOT Form No. 500-065-19) for the period of reimbursement.
Number 21. Publication and Printing of Observational Surveys and Other Reports - The
verbiage in Part V has been modified to clarify publishing requirements.
Number 29. Public Information and Education Items - The verbiage in Part V has been
modified to add NHTSA's descriptions of items and to add references to NHTSA Memo "Use of
NHTSA Highway Safety Grant Funds for Certain Purchases" (dated May 18, 2016), showing
that the use of NHTSA grant funds to purchase promotional items or memorabilia is
prohibited and therefore unallowable under this subgrant.
• Number 32. Personnel Hired Under Agreement - The verbiage in Part V has been modified to
clarify that personnel funded under the subgrant shall not hold the position of Project Director
nor receive any benefit under the grant.
• Number 33. Overtime - The verbiage in Part V has been modified to clarify overtime hours
and related requirements.
• Number 39. Certification for Equipment Costing More than $1,000 per item - The
requirement to provide a certification letter has been removed; however, it is clearly noted
that Subgrant funds shall not be used to replace previously purchased equipment that is
damaged, stolen, lost, or that wears out as a result of misuse, whether the equipment was
purchased with federal, state, or local funds.
• Number 40. Vehicles - The verbiage in Part V has been modified to include the requirement
of the Safety Grant Vehicle Use Form (FDOT Form No. 500-065-21) and the Safety Grant
Equipment Maintenance Form (FDOT Form No. 500-065-22) with each request for
reimbursement.
• Number 46. Impaired Driving Enforcement, Training and Reporting - The verbiage in Part V
has been modified to clarify the training requirements.
