Artistic Awakening, INC is a small organization and therefore has special controls in place to
ensure strict financial management. The corporate Chairperson is in constant contact with the
organization, receives ongoing financial reports, and personally reviews and signs all forms
relating to grants from governmental funding agencies. Our accountant, Mr. Nicholas Alvarez,
CPA, works for Alvarez & Alvarez, P.A. - a firm that specializes in accounting for non -for -profit
organizations. Mr. Alvarez regularly reviews the books of the corporation, advises on improving
and enhancing bookkeeping practices, reviews payroll tax forms, and prepares an annual Financial
Compilation.
Artistic Awakenings, Inc., a non-profit 501(c) (3) community -based organization, is requesting
funding for fiscal year 2016-2017 in the amount of $25,000 to operate the Artistic Awakening
Dorothy Quintana Senior Arts Program for the Wynwood community's senior population.
Wynwood, a community within the City of Miami, is an area traditionally known for having little
or no economic resources. The National Objective for this activity is Assistance to Low/Moderate
income persons as defined in 24 (CFE) 527.208 (a) (2). The HUD Matrix Code is 05A - Senior
Services.
The activity will take place:
• Within the Wynwood Neighborhood Development Zone (bordered on the north by NW 36th
Street, the east by North Miami Avenue, the south by NW 29th Street, and the west by I-95,
• the Model Block area (NW 36th Street to the south, North Miami Avenue to the east and NW
4th Avenue to the west- Census block group 26003 and 26002),
• the Wynwood Commercial Corridors: (a)Wynwood NW 2nd Avenue Corridor (Old San Juan
Cultural and Entertainment District;(b) NW 2nd Avenue from 34th Street to the south to 38th
Street to the north; and (c) NW 2nd Avenue from 14th Street to NW 39th Street.
Artistic Awakenings, Inc. intends to serve a minimum of 20 seniors through its Dorothy Quintana
Senior Arts Program. The goal of this program is to provide basic arts and crafts instruction to
enable seniors to discover - or rediscover their artistic talents and abilities. The program provides
seniors with an opportunity to break the monotony of their daily routines, their isolated lifestyles,
and become involved in a socially interactive and mentally stimulating activity. Studies have
shown that seniors, who remain active in such mentally stimulating and creative activities as arts
& crafts delay the onset of various forms of dementia and other health issues.
Arts & Crafts Classes will introduce the target population of low income seniors to the various
genres of art (i.e. embroidery, knitting, textiles, Papier-Mache, painting, etc.) These activities will
be taught by individuals with varied artistic skills and talents. At present, we have one instructor
who has been provided to the program through an in -kind donation. Upon receipt of funding,
Artistic will hire additional instructor(s). Additionally, Artistic Awakening will provide guest
instructors on a volunteer basis to provide instruction on their individual areas of expertise.
Survey and Marketing: We will conduct a survey of the area's population. The survey will
introduce the program to neighborhood seniors who are currently unaware of the activities at the
Dorothy Quintana Community Center. For those seniors who express an interest in the program,
we will immediately begin the paperwork and processing to ensure their eligibility for the program.
All seniors will be invited to attend and participate within the Senior's Art Shows, Health Fairs,
and other special events sponsored by the program and our community collaborators, Borinquen
Medical Centers of Miami Dade.
Arts & Crafts: In each session, the art instructor will engage the seniors in developing their
potential using various artistic materials. In addition to traditional arts & crafts materials and
supplies, there will be an emphasis placed upon recycling - using bottles, jars, newspapers, boxes
and other recyclable materials.

Artistic Awakenings, Inc. has established quarterly benchmarks to ensure that the Dorothy
Quintana Senior Arts Program meets its goals within the time frame of the grant from the City of
Miami. The staff of Artistic Awakenings, Inc.. will meet regularly to discuss the progress of each
aspect of the program. In addition to meeting its goals, the agency will seek to determine the impact
the program is having upon individual seniors, the community and the service area as a whole.

Artistic Awakening's corporate offices are located at 3550 Biscayne Blvd., Ste.
304. Program delivery will occur at the Dorothy Quintana Community Center
within Roberto Clemente Park - 101 NW 34th Street.
