ALFONSO City Manager CENTER BRICKELL SUBDIVISION TENTATIVE PLAT 11828 A (FIRST RESUBMITTAL) LOCATED BETWEEN SOUTH MIAMI AVENUE AND 13 CKELL AVENUE, AND BETWEEN SE 1.3 STREET AND SE 14 STREIT. Ladies and Cientlemen: The City of Miami Plat and Street Committee, at its meeting of November 5, 2015 approved the above tentative plat subject to the following revisions being made to the tentative plat, additional information being provided and/or variances being granted. Please be advised that the processing of your tentative plat cannot proceed until these conditions have been satisfied: 1. On the Location Sketch, show the Lot and .Block numbers, SubdivisionSubdiv.ision names, and Plat Book and Page numbers of the underlying plat and surrounding subdivisions. 2. On the application: a) Provide corporate resolution for Tigervest NV. b) The property owner is an off -shore corporation. Provide a financial statement, alien report, who has authority to bind (managing director) and Right to do business in U.S. or Florida. c) Provide witnesses signatures. Please use the most current application form. d) If the corporation has a corporate seal, provide seal in the application. e) This is a foreign company. Provide the documents required to execute the tentative plat. f) Provide developer information for Tigervest NV. g) Provide information for item No.3 for Tigervest NV. h) Correct application form for Pacific National Bank. Use the corporation form, not an individual form. i) Correct the zoning on the Tigervest NV application. 3, An approval letter from F.D.O.T. will be required prior to the final plat submittal. 4. Expand the statement "AN EXPRESSED .PURPOSE OF THIS PLAT IS TO CLOSE, VACATE AND ABANDON FROM PUBLIC USE TF1AT POIMON OF LEE Al.,LEY LYING WITHIN THE LIMITS OF THIS PLAT," 5. Expand the Development Intent, and provide a purpose statement. 6. If the existing structures are to remain, verify they meet zoning regulations. 7. Show the areas to be dedicated as public right of way by the proposed plat by using a solid line for the existing Right of Way lines and a dashed line for the zoned Right of Way lines, 8. Provide a Certified Corner Record. 9. A letter from Comeast is required to determine if any adjustments to their facilities or easenients are required, 10. Verify with Miailli-Dade County that the proposed plat name is acceptable. An opinion of title, in the City of Miarni opinion of title form, must be provided at the time of the final plat submittal. 1.3. Current backup documentation will be required for all who execute the final plat. A resolution for authority to execute documents and a Certificate of Good standing from. the Secretary of State are required, if applicable. 1.4. Tentative plat application must be made with Miami -Dade County after receiving approval from the City of Miami Plat and Street Committee. 1.5. Be advised that an incomplete final. plat package will not be accepted by the City of Miami. It is the owner's responsibility to work with. his surveyor and his attorney to assure that everything is in order before submitting the final plat package. 1.6. Be advised that if allrequirements for scheduling the final plat for City Commission action are not in order, the final. plat will not be scheduled for a City Commission meeting. 17. Provide a tree survey and table including common and botanical names, .height, diameter and spread certified by an Arborist or Landscape Architect. 18. A letter from the Miami — Dade Water and Sewer Department is required to determine if any adjustments to their facilities or easements are required. 19. The planning, Zoning and Appeals Board required a sketch and legal description with a square footage, on an 8 x 1.1" paper, for the proposed closure. Provide a copy to the Public Works Department. 20. Confirm the Official Record Book and Page of all recorded easements. 21. A Waiver from the Office of Zoning may be required for pavement across the proposed Tract Line, 22. Provide a copy of the alley closure Resolution No. R-I4-0246. 23. Correctly label "South Miami Avenue"; delete reference to "NE 1 Avenue" on the location sketch, 24. Provide the FEMA. flood map information on the main sketch. 25. Remove proposed. Tracts from the location sketch and provide only the underlying plat information. 26. Revise the zoning designation on the, location sketch in accordance with the City of Miami GIS webs ite 27. Clarify the zoning district boundaries on the location sketch. 28. Provide a Zoning Verification Letter approving the Tentative Plat and provide a copy of any Unity of Title, if applicable. 29. Provide an analysis to determine that the existing structures comply with Miami 21 Zoning code regulations. Provide a copy of Microfilm records to the Zoning Administrator. 30. For development information, be aware Article 5, Section 5.6.1.i of the Miarni 21 Zoning Code may require a cross -block pedestrian passageway. 31.. Section 55-15 (h) requires that the approved final plat must be recorded within four years of the approval of the vacation and closure of the alley. In addition to the above requirernents, you should be aware of the following: 1. State and local laws require th.e installation of various physical improvements in the public rights -of -way when property is platted. These subdivision improvements include paving, drainage, landscaping, sidewalks, etc. In some cases this could represent a substantial investment on your part. REQUIRED: The submission of an electronic copy of the tentative plat; in an AutoCAD version, to the ,City of Miami Public Works Department, Roadway Plans Section will EXPEDITE the preparation and enhance the ACCURACY of the subdivision improvement letter required for the final plat approval. 2. The alteration, reloeation or installation of utilities such as storm and sanitary sewers, electric, telephone, water, etc., caused by this plat will be at the property owner's expense. Also, utility easements may be required on the property being platted. 3. A building permit will not be issued on the property being platted until the final plat is recorded or as authorized by the City of Miami Code Section 55-10(i). Also, the Certificate of Occupancy for any building construction will be issued only after all the required subdivision improvements have been completed. 4. Approval for fire flow requirements must be obtained from the Fire -Rescue Department prior to the issuance of a building permit. In order to mitigate problems associated with access and construction activities throughout the neighborhood, the contractor/developer is encouraged to notify the affected neighborhood residents, in writing, of the project starting and eompletion dates at the time of issuance of permits. The contractor/developer may coordinate the notification of residents with the local City of Miami NET Service Center. 6. Additional items must be provided to the City of Miami Department of Public Works before the final plat is submitted to the City COMMiSSiOrl for approval. You will he notified in writing as to what these items are after the amount of the bond has been determined for the necessary subdivision improvements. It is required that the applicant contacts the City of Miami Public Works Survey Section to verify that all final plat submittal documents are complete prior to final plat submittal. 7. Tentative plat approval is only valid for one (I) year and six (6) months from the date of the Plat and Street Committee meeting at which time it was 'approved.
