STATEMENT OF EXTRA -CURRICULAR ACTIVITIES AND/OR COMMUNITY SERVICE / AWARDS DEADLINE TO SUBMIT ALL ITEMS ABOVE IS Friday, April 29, 2016 (Note: Extended to Wednesday, May 18, 2016) City of Miami, Department of Solid Waste Educational Trust Fund Scholarship Selection Criteria CHILDREN AND/OR LEGAL DEPENDENTS OF SOLID WASTE EMPLOYEES APPLICATION Minimum Criteria: • Parent or legal guardian of applicant must be employed as a full-time permanent employee with the Department of Solid Waste for a minimum of two (2) years, and shall be in good standing with the Department of Solid Waste ("Qualified SW Employee"). "Good standing" shall mean that the SW employee has no written disciplinary actions issued within one (1) year from the date of application. Separation of a Qualified SW Employee from the Department of Solid Waste, excluding layoff, will automatically terminate any obligation to fund any scholarship awarded to said employee's children and/or legal dependents. The children and/or legal dependents of employees laid off in accordance with the Civil Service Rules shall be permitted to apply for the scholarship for one (1) additional year. Legal dependent, for purposes of the scholarship, does not include spouses but rather dependent children up to the age of twenty-six (26). Further, for purposes of the scholarship, a dependent "child" must be a birth child, a legal stepchild, or a legal foster child in the employee's care. • Applicant must be a legal U.S. resident, has a high school diploma or its equivalent, and must not be over the age of twenty-six (26) at the time of application. • Applicant must be enrolled as a full-time student, taking a minimum of 12 credits, at an accredited educational institution (Le., university, college, vocational, or technical school). • Applicant must have a minimum unweighted grade point average of 2.5. • Applicant must submit the following .by Friday, April 29, 2016 (Note: Extended to Wednesday, May 18, 2016) in order to receive consideration by the Selection Committee: o Completed Application Form; o A statement of extra -curricular activities, including community service projects, and other forms of involvement; o A 500-word essay on a topic determined by the Selection Committee annually; o Proof of age for dependent child. Card, Driver's License, Birth Certificate; o Official sealed transcript reflecting the applicant's unweighted grade point average; and o Two (2) Letters of Recommendation ■ A community leader or clergy on letterhead ■ A teacher, counselor or administrator on school letterhead 1 Written disciplinary actions shall consist of the following: Records of Formal Counseling, Written Reprimands, Letter of Suspension, or Letter of Termination. City of Miami, Department of Solid Waste Educational Trust Fund Scholarship Selection Criteria Selection Process: • The Selection Committee shall review all applications submitted, and shall recommend scholarship awards to the Director of the Department of Solid Waste. • The Selection Committee shall weigh applications in the following manner: o A maximum of forty (40) points may be assigned to an unweighted grade point average as follows: ■ Applicants with a 2.5 GPA to 2.9 GPA shall be assigned twenty-five (25) points. ■ Applicants with a 3.0 GPA to 3.4 GPA shall be assigned thirty (30) points, ■ Applicants with a 3.5 GPA to 3.9 GPA shall be assigned thirty-five (35) points. ■ Applicants with a 4.0 GPA shall be assigned forty (40) points. o A maximum of twenty-five (25) points may be assigned to the applicant's extra -curricular activities. o A maximum of twenty-five (25) points may be assigned to the applicant's essay. o A maximum of ten (10) points may be assigned to the applicant's letters of recommendation. o BONUS: Five (5) points given to past award recipients (awarded in the last 2 years). Award Information: • Maximum annual scholarship shall be Three Thousand Dollars ($3,000.00) per student and shall be awarded no more than four (4) years per student. • Scholarship will only be awarded after proof of official registration is received for Fall Semester. • Scholarships shall be applied solely towards the cost of tuition, and shall not be used for any other purpose. • If awarded, monies will be deposited with the accredited institution. Any portion of the award not utilized shall be returned to the City of Miami Educational Trust Fund. No monies shall be paid directly to the student or parent/legal guardian. • Award of a scholarship to an applicant does not prevent said applicant from applying for the same scholarship in a subsequent year. However, award in a subsequent year is not automatic, and will require the applicant to meet the eligibility criteria. Temporary employment status in the Department of Solid Waste for up to one (1) year may be used to meet the minimum years of service. "Good standing" shall be mean that the SW employee has no written disciplinary actions' issued within one (1) year from the date of application. Separation of a Qualified SW Employee from the Department of Solid Waste, excluding layoff, will automatically terminate the City's obligation to fund any scholarship awarded to said employee. Employees laid off in accordance with the Civil Service Rules shall be permitted to apply for the scholarship for one (1) additional year. • Applicant must have a high school diploma or its equivalent at the time of application. • Applicant must be enrolled as a student at an accredited educational institution (i.e., university, college, vocational, or technical school) and taking a minimum of 6 credits. In the event the number of applications exceeds the allotted number for the fiscal year, the applicants will be selected based on seniority in the department. • The Selection Committee shall weigh applications in the following manner: o A maximum of fifty (50) points may be assigned to the applicant's essay. o BONUS: Five (5) points given to past award recipients, provided that the recipient has obtained a minimum grade point average of 2.5. o BONUS: Ten (10) points given if applicant has participated in several community service and/or church activities; and/or showed commitment to a specific cause. Temporary employment status in the Department of Solid Waste for up to one (1) year may be used to meet the minimum years of service. "Good standing" shall mean that the SW employee has no written disciplinary actions1 issued within one (1) year from the date of application. Separation of a Qualified SW Employee from the Department of Solid Waste, excluding layoff, will automatically terminate the City's obligation to fund any scholarship awarded to said employee.
