Subject: Accessing — Annual Firefighter Physicals, RFP 13-407; Polk County
Purpose of Item:
The nature of this item is to establish a resolution authorizing access to utilize the existing Polk County Annual Firefighter
Physicals Contract No. 13-407, pursuant to Section 18-111 of the Code of the City of Miami, with Life Extension Clinics,
Inc. which was competitively solicited for three years and will expire on September 30, 2016, with two, one year automatic
renewal periods, subject to any extensions and/or replacement contracts by Polk County; allocating funds from the
Department of Fire -Rescue (Fire) General Fund, subject to availability of funds and budgetary approval at the time of need.
Background Information:
Fire has identified a need for a comprehensive health physical that focuses onearly detection and prevention for public
safety officers. Accessing of the Polk County Contract No. 13-407 will enable Fire to utilize an approved vendor with the
necessary equipment and personnel to provide the full body scan. This is a three year contract that will expire on September
30, 2016 with two, one year automatic renewal periods. The estimated fiscal impact is $284,620 for the initial service period
that ends September 30, 2016. Subsequent fiscal impact is dependent upon firefighters tested which is based upon previous
results, years of service, age, and position, but estimated to be no more than $568,520.
