TYPE OF BENEFIT PROPOSED BENEFITS SALARY $137,500 annual salary (payable bi-weekly) Effective December 10, 2015 Car Allowance $300 per month Cellular Phone Allowance $100 per month INSURANCE HealthNision/Dental Life Accidental Death & Dismemberment Insurance • Disability Insurance Executive Benefit Plan 2X Annual Salary 2X Annual Salary In the event that a disability plan is offered to other City executives, the City Clerk shall be provided a similar benefit. EXECUTIVE PHYSICAL One annual physical LEAVE TIME Vacation Sick Holidays 20 days per year, with a maximum of 15 days to be taken consecutively. 10 days per year 11 days per year Upon honorable separation, the City Clerk shall receive payment of all Vacation and Sick leave earned as of the Effective Date of his original Contract dated 1/15/2013 (earned Vacation and Sick leave will be paid out at the hourly rate associated with Mr. Hannon's position as Assistant City Clerk as of 1/14/2013). Vacation and Sick time earned after the Effective Date of the original Contract that is not utilized during the Term shall have no "cash value." Such Vacation and Sick time shall be available on a use -it -or -lose -it basis over the duration of the Term. SEVERANCE NONE SALARY INCREASES Same as granted to the members of the classified or unclassified service, whichever is higher. PENSION City of Miami Pension Plan.
