Can the square footage floor for the Mobilization e on line 1.22 be increased from 10 square feet to 1000 square feet or can it be for all jobs regardless of size? A2. The City will agree to increase the minimum from 10 square feet to 250 square feet. Therefore any job less than 250 square feet is allowed a mobilization fee. Refer to line 1.22. Q3. Materials purchased by the contractor are subject to sales tax even though the City is tax exempt. Can we mark up those invoices a specified amount to cover those taxes? A3. No markup will be accepted, The City of Miami's tax exemption certificate is attached. Page 1 This is requred only for Community Development Projects SECTION 3 & MBENVBE/DBE CLAUSE A. The work to be performed under this contract is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended, 12 U.S. C. 1701u (Section 3). The purpose of section 3 is to ensure that employment and other economic opportunities generated by HUD assistance of HUD -assisted projects covered by Section 3, shall, to the greatest extent feasible, be directed to low- and very low-income persons, particularly persons who are recipients of HUD assistance for housing, B. The parties to this contract agree to comply with HUD's regulations in 24 CFR part 135, which implement section 3. As evidenced by their execution of this contract, the parties to this contract certify that they are under no contractual or other impediment that would prevent them from complying with the part 135 regulations. C. The contractor agrees to send to each labor organization or representative of workers with which the contractor has a collective bargaining agreement or other understanding, if any, a notice advising the labor organization or workersrepresentative of the contractor's commitments under this Section 3 clause, and will post copies of the notice in conspicuous places at the work site where both employees and applicants for training and employment positions can see the notice. The contractor agrees to include this Section 3 clause in every subcontract subject to compliance with regulations in 24 CFR part 135, and agrees to take appropriate action, as provided in an applicable provision of the subcontract or in this section 3 clause, upon a finding that the subcontractor is in violation of the regulations in 24 CFR part 135. The contractor will not subcontract with any subcontractor where the contractor has notice or knowledge that the subcontractor has been found in violation of the regulations in 24 CFR part 135. E. The contractor will certify that any vacant employment positions, including training positions, that are filled (1) after the contractor is selected but before the contract is executed, and (2) with persons other than those to whom the regulations of 24 CFR part 135 require employment opportunities to be directed, were not filled to circumvent the contractor's obligations under CFR part 135. F. Noncompliance with HUD's regulations in 24 CFR part 135 may result in sanctions, termination of this contract for default, and debarment or suspension from future HUD assisted contracts. A. MBE/WBE/DBE (i) The Development Partner's selection policy and procedures shall foster the utilization of minority -owned businesses and women -owned businesses. A minority business enterprises ("MBEs") and women business enterprises ("VVBEs") combined utilization goal of not less than twenty-five percent (25`)/0) of the total contract value shall be required on all contracts. (ii) The Development Partner shall comply with Section 3 of the Housing and Urban Development Act of 1968 and the implementation regulations at 24 C.F.R. part 135 (the "Section 3 Plan"). The purpose of Section 3 is to ensure that employment and other economic opportunities generated by HUD assistance shall be, to the greatest extent feasible, directed to low and very low-income persons, particularly those who receive HUD assistance for housing. (iii) The Development Partner shall formulate plans for MBEs and WBEs participation and Section 3 Hiring on terms approved by the Authority. The Hiring and Training Plans shall specify how affirmative action and resident hiring requirements shall be imposed, how the Authority's minority goals shall be met, and how all requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended, shall be met, with respect to the Development in accordance with the Section 3 Plan and the Section 3 plans, programs and policies of the Authority. The Hiring and Training Plans shall be applicable to services rendered by the Development Partner, any Owner Entity and their respective contractors and subcontractors. The Development Partner shall implement or cause the Owner Entities to implement the Hiring and Training Plans, as approved by the Authority. (iv) The Development Partner shall make affirmative outreach efforts to publicize training, employment and subcontracting opportunities. Notice shall be put in local newspapers and flyers shall be distributed as appropriate. Local unions, elected officials and training organizations shall also be notified. The Development Partner's efforts under the Section 3 Plan shall be documented monthly, and as evidenced by the execution of this Agreement, the Development Partner certifies that it is under no contractual or other impediment that would prevent compliance with 24 CFR Part 135. B. MBE and WBE Participation. To achieve greater participation of MBEs and WBEs in contracts administered directly or indirectly by the Authority pursuant to Executive Orders 11625 and 12138, the Developer agrees to use its good faith best efforts to: (1) place qualified MBEs and WBEs and small business concerns on solicitation lists; (2) divide the Development Services into smaller tasks or quantities to permit maximum participation by MBEs and WBEs and small business concerns; (3) use the services and assistance of the U.S. Small Business Administration, the Minority Business Development Agency of the U.S. Department of Commerce, any local minority assistance organizations and various state arid local government small business agencies; (4) comply with such additional requirements relating to MBEs, WBEs, and Section 3 as set forth in the hiring and training plans developed by the Owner Entity, and approved by the Authority (the "Hiring and Training Plans") C. MBEs/WBEs Report. Each quarter, the Developer shall, and shall cause each Subcontractor to prepare and submit to the Owner Entity the Section 3, MBEs and WBEs report in the form prescribed by the Owner Entity. D. Section 3. The Developer shall, and to the extent practicable shall require its Subcontractors to, ensure that employment opportunities shall be directed to: (1) the Authority's public housing residents, then (2) low income residents of Miami Dade County in accordance with the Hiring and Training Plans. The facility exhibited heavy metal contamination that warranted extensive abatement efforts and MCO Environmental was awarded the contract for several reasons. MCO responded to the request for proposal in a timely manner and addressed all the items in the RFP in a thorough and clear proposal. Workers shall utilize MSHA/NIOSH half face dual cartridge respirators equipped with HEPA cartridges for all pre -cleaning, prepping and removal activities. Workers shall wear double disposable full body coveralls. Before leaving the work area, the worker shall thoroughly vacuum the outer disposable coverall and dispose of it as ACM contaminated material. The inner suit will be cleaned and disposed in the same manner. PROGRAM EVALUATION The Program Administrator will periodically assess the effectiveness of the Respiratory Protection. Program during all phases of operation in which respirators are being used. Frequent walk-through inspections during these activities will be conducted to monitor and document Supervisor and worker compliance with the requirements of the program. In addition to general assessment of the overall respirator cleaning, inspection, maintenance, repair, storage and use procedures will be frequently conducted to ensure that the desired results of these operations are consistently achieved. VIOLATION AND DISCIPLINARY ACTION Due to MCO Environmental, Inc., heavy involvement in the asbestos abatement industry, respirator protection is a crucial part of the Company's overall safety program. As such, mandatory compliance with all aspects of this program by those employees required to use a respirator is a condition of continuing employment. DISCIPLINARY ACTION When it has come to the attention of a Supervisor that an employee has deliberately removed his/her respirator or broken the face piece to face seal while in the contaminated area, the employee will be immediately suspended from work and instructed to leave the job site pending a final disposition. Randomspot checks will be conducted to determine the effectiveness of the employee's fit test. Should the check, which will be a positive or negative pressure test conducted under the direction of a Supervisor, indicated that the employee's respirator does not have a satisfactory seal, the employee will be advised accordingly and instructed to leave the contaminated. area. A written citation will be issued to the employee the first time he/she fails a random check. Such citations will be sufficient cause for dismissal. NOTE: Should any employee request permission to leave the contaminated area to remove his/her respirator for the purpose of washing either the face piece of their face, this permission must be granted. However, the employee should be reminded that the positive and/or negative pressure test must be conducted before returning to the contaminated area. REPORTING RESPIRATOR. PROBLEMS Occasionally, the Company may find a defect in the design or performance of a respirator. The course to follow is to report these findings of the Administrator of the Company's Respiratory Protection Program. If the respirator carries with it the approval of the Mines Safety and Health Administration (MSHA) and the National Institute for Occupational Safety will report the findings .to the respirator's manufacturer and to NIOSH. This is done by notifying the manufacturer of the defect in a report format and forwarding a copy of the report to NIOSH. The report will include the following: ▪ The name, address and telephone number of MCO Environmental, Inc. The name of the respirator's manufacturer. The model number of the respirator. • The name and part number (if possible) of the defective part. A 'brief description of the respirator's use when the defect was discovered. • A description of the defect. • A description of the defect's adverse effect on the respirator's performance. This report should be addressed to the NIOSH Division of Safety Research, testing and certification branch, 944 Chestnut Ridge Road, Morgantown, West Virginia 26595. A medical and work history with special emphasis directed to the pulmonary, cardiovascular, and gastrointestinal systems. B. Completion of the "Initial Medical Questionnaire" during initial examinations, and the "Periodic Medical Questionnaire" during annual examinations. C. A physical examination directed to the pulmonary and gastrointestinal systems, including a chest roentgenogram to be administered at the discretion of the physician, and pulmonary function tests of forced vital capacity (FVC) and forced expiratory volume at one second (FEVI). Interpretation and classification of chest roentgenogram will be conducted according the following: 1. Chest roentgenogram will be interpreted and classified in accordance with a professionally accepted classification system and recorded on a roentgen graphic interpretation form (Form CSD/NIOSH (M) 2.8). 2. Roentgenogram will be interpreted and classified only by a B-Reader, a board eligible/certified radiologist, or an experienced physician with known expertise in pneumoconiosis. All interpreters, whenever interpreting chest roentgenogram made under this section must have immediately available for reference a complete set of the ILO-U/C International Classification of Radiographs for Pneumoconiosis, 1980. 11. Information provided to the physician A. A description of the affected employees' asbestos related duties. B. The employee's representative exposure level or anticipated exposure level. C A description of any personal and respiratory protective equipment used or to be used. D. Pertinent information from previous medical examinations of which the examining physician may be unaware. E Medical Surveillance Guidelines 4 ROUTE OF ENTRY :Inhalation, Ingestion 2. TOXICOLOGY Clinical evidence of the adverse effects associated with asbestos, tremolite, anthophyllite, and actinolite, is present in the form of several well-conducted epidemiological studies of occupationally exposed workers, family contacts of workers, and persons living near asbestos, tremolite, anthophyllite, and actinolite mines. These studies have shown a definite association between exposure to asbestos, tremolite, anthophyllite, and actinolite and an increased incidence of lung cancer, pleural and peritoneal mesothelioma, gastrointestinal cancer, and asbestosis. The latter is a disabling fibro tic lung disease that is caused only by exposure to asbestos. Exposure to asbestos, trem.olite, anthophyllite, and actinolite has also been associated with an increased incidence of esophageal, kidney, laryngeal, pharyngeal, and buccal cavity cancers, as with other known chronic occupational diseases, disease associated with asbestos, tremolite, anthophyllite, and actinolite generally appears about 20 years following the first occurrence of exposure. There are no known acute effects associated with exposure to asbestos, tremolite, anthophyllite, and actinolite. Epidemiological studies indicate that the risk of lung cancer among exposed workers who smoke cigarettes is greatly increased over the risk of lung cancer among non -exposed smokers or exposed nonsmokers. These studies suggest that cessation of smoking will reduce the risk of lung cancer for a person exposed to asbestos, tremolite, antholphyllite, and actinolite but will not reduce it to the same level of risk as that existing for an exposed worker who has never smoked. Signs and symptoms of exposure -related disease the signs and symptoms of lung cancer or gastrointestinal cancer induced by exposure to asbestos, tremolite, anthophyllite, and actinolite are not unique, except that a chest t x- ray of an exposed patient with lung cancer may show pleural calcification, or pleural fibrosis. Symptoms characteristic of mesoth.elioma include shortness of breath, pain on the walls of the chest, or abdominal pain. Mesothelioma is therefore more likely to be found among workers who were first exposed to asbestos at an early age. Mesothelioma is always fatal. Asbestosis is pulmonary fibrosis caused by the accumulation of asbestos fibers in the lungs. Symptoms include shortness of breath, coughing, fatigue, and vague feelings of sickness. When the fibrosis worsens, shortness of breath occurs even at rest. The diagnosis of asbestosis is based on a history of exposure to asbestos, the presence of characteristic radiologic changes, end - inspiratory crackles (rales), and other clinical features of fibrosis -producing lung disease. Pleural plaques and thickening may also be observed on x-rays. Asbestosis is often a progressive disease even in the absence of continued exposure, although this appears to be a highly individualized characteristic. In severe cases, death may be caused by respiratory or cardiac failures. 5 3 SURVEILLANCE AND PREVENTIVE CONSIDERATIONS As noted above, exposures to asbestos, tremolite, anthophyllite, and actinolite has been linked to an increased risk of lung cancer, mesothelioma, gastrointestinal cancer, and asbestosis among occupationally exposed workers. Adequate screening tests to determine an employee's potential for developing serious chronic diseases, such as cancer, from exposure to asbestos, tremolite, anthophyllite, and actinolite do not presently exist. 111, Physicians Written Opinion A. The physician's opinion as to whether the employee has any detected medical conditions that would place the employee at an increased risk of material health impairment from exposure to asbestos. B. Any recommended limitations on the employee or on the use of personal protective. C. A statement that the employee has been informed by the physician of the results of the medical examination and of any medical conditions that may result from asbestos exposure. RESPIRATORY TRAINING PROGRAM OUTLINED SELECTION OF RESPIRATORS Selection of respirators is based upon the type of Asbestos Abatement work being performed. The following is a brief description The employees have received training in the use of the respirators. Sergio Otazo, Compliance .Manager -Health Manager, conducted this training. Attached is an outline of training. We also provided training to employees who might be associated with an emergency condition. Attached is a list of all employees who have received training and the dates they received their training. There is a random inspection of areas where respirators are used to see that employees are using the respirators properly and have not developed any conditions that interfere with the function of their respirator. INSPECTIONS ARE MADE ON A WEEKLY BASIS OF RESPIRATORS AS WELL AS EMERGENCY CONDITIONS. A RECORD IS MAINTAINED OF THE DATE AND THEIR. FINDINGS. 6 'This policy serves as a guiding principal for the preparation, implementation, anci enfbrcementof an. effective Respiratory Protection Program. The training in this program will provide a detailed discussion of the use, maintenance and limitations of respiratory protection. Respiratory System There are three ways that hazardous materials can enter the body; 1. Through the Gastrointestinal tract, usually •through the mouth. 2. Through the skin. 3. Through the respiratory systems. NOTE: Asbestos does not appear to pose a serious threat to the body through the first or second routes of entry. It can, however, cause serious diseases when it enters the body through the respiratory system. B. Respiratory Hazards on the Job Respiratory Hazards are generally divided into two categories: 1. Toxic Contaminants 2. Oxygen Deficiency Generally, Asbestos Abatement projects do not pose oxygen deficiency hazards, but it must always be considered. (example: Steam Tunnels, Mechanical Chases or Boiler Room.) Toxic Contaminants are a more common category of Respiratory Hazards. These toxic contaminants are generally divided into three categories: 1. Particulate 2. Gaseous materials 3. Vapors Or a combination of two or all of the above. Asbestos fibers are an example of the Particulate category, Carbon Monoxide is an example of the Gaseous category, and an Epoxy Encapsulant is an example of a harmful organic vapor. C. The Selection of Appropriate Respiratory Equipment A. The respirator selected must conform to O.S.H.A. standards and guidelines published by respiratory manufacturers. (29 CFR 1910.134) Requires that only approved respirators be used, and they must be approved for protection specifically against Asbestos fibers. 7 The National Institute For Occupational Safety and Health (NIOSIl) is the official testing and approval agency for respirators. If all of the accessories pass the NIOSII test, then they issue a NIOSH approval number. The specific number is preceded by the letters "TC", which indicates the respirator assembly was "Tested. and Certified". IV. Fit -Testing A. The employee shall be trained on how to put a respirator on. After the employee has adjusted the respirator for the comfort of this face, at least 1.0 minutes, then he goes on to perform a qualitative test which consists of the person to run in place, talking, head movements the method to be used for qualitative fit .test will be an irritant smoke test-. 8 .B. For Half Face respirators after the employee has selected his respirator and no visual leaks are evident a negative pressure check and pressure check are performed. 1 Negative Pressure Check The employee closes off the inlet of the filter or cartridges by covering them with the palms of the hands or by squeezing the breathing tube so that the air cannot pass through, inhales so that the face piece collapses slightly, and holds his/her breath for about 10 seconds. 2. Positive Pressure Check It is conducted by closing off the exhalation valve of the respirator and gently exhaling into the face piece. The respirator fit is considered passing if the positive pressure can be built up inside the face piece without evidence of outward air leakage around the face piece. VI. Inspecting the Respirator All respirators are inspected before and after each use and inspected at least monthly. Irritant Smoke: The wearer must don a respirator equipped with .FIEPA (high efficiency) filters. 2. The wearer must close his/her eyes tightly, 3. The tester must break the ends of the smoke .tube and insert one end of the tube into the squeeze bulb. 4. Squeeze the bulb to create a cloud of sm.ok.e near .the wearer's face. The wearer should run through the exercises listed below for 1.5 seconds apiece, If he/she coughs, sneezes, complains of irritation, try repositioning the headband and face piece to eliminate the leak. 5. Respirators and filters shall be selected at no cost to employees on the basis of hazard exposure and protection to the worker. Perform either quantitative or qualitative face fit tests at the time of initial fitting and at least every 6 monthsthereafter for each employee wearing a negative -pressure respirator. The qualitative fit tests may be used only for testing the fit of .half face mask respirators where they are permitted to be worn, or of fuJi facepiece air purifying respirators where they are worn at levels at which .half facepiece air purifying respirators are permitted. c. User Instructions. The employees assigned respirators shall be trained in the proper use and the limitations of the respirators that they are assigned. d. Issuance. The respirator with the appropriate filters shall be given to the worker free of charge. They must be properlysized, and fitted to conform to the fa.ci.a1. characteristics of the user. The respirator and/or the filters shall be immediately changed whenever they fail to work properly. e. Maintenance, Cleaning, & Storage. Respirators shall be maintained in good working conditions at all times. Respirators shall be regularly cleaned and disinfected. Those used by more than one worker shall be stored in a convenient, clean, dry, and sanitary location.. Inspection. Respirators used routinely shall be inspected during cleaning. Worn or deteriorated parts shall be replaced with the original manufactures' approved parts. Respirators for emergency use such as self-contained devices shall be thoroughly inspected at least once a month and before and after each use. Monitoring. Appropriate monitoring and surveillance of work area conditions and degree of employee exposure or stress shall be maintained. There shall be regular inspection and evaluation to determine the continued effectiveness of the program. Persons should not be assigned to tasks requiring use of respirators unless it has been. determined that they are physically able to perform the work and use the equipment. The local physician shall determine what health and physical conditions are pertinent. Th.e respirator user's medicalstatus should be reviewed periodically (at a minimum, annually). 2 ol Respirators shall be selected from among those jointly approved by the Mine Safety and Health Administration and the .National Institute for Occupational Safety and Health under the provisions of 30 CFR part 11.. h. Planning for Emergencies and Rescue. Communications (visual, voice, or signal line) shall be maintained between all individuals present. Planning shall be such that one individual will be unaffected by any likely incident and have the proper rescue equipment to be able to assist the other(s) in case of emergency. 2. Facial .Hair and Other Prevention.s. Respirators shall not be worn when conditions prevent a good face seal. Such conditions may be a growth of beard, sideburns, a skull. cap that projects under the facepiece, or temple pieces on glasses. Also, the absence of one or both dentures can seriously affect the fit of a facepiece. The worker's diligence in observing these factors shall be evaluated by periodic check. To assure proper protection, the facepiece sit shall be checked by the wearer each time he puts on .the respirator. This may be done by following the manufacture's facepiece fitting instructions. If corrective specialties or goggles are required, they shall be worn so as not to affect the fit of the facepiece. Proper selection of equipment will minimize or avoid this problem. 3. Prohibit Use of Compressed'Air and Other Types of Gases. Respiratory devices using compressed air, compressed gaseous oxygen, liquid air, or liquid oxygen are prohibited unless approved prior to .use by a qualified person. . Procedures to Enter or Working in Atmosphere Immediately Dangerous to life or Health (IDLH). Employees entering or working in atmospheres which are immediately dangerous to life or health (IDLH) shall wear either a self-contained breathing apparatus of the pressure demand type open -circuit or positive —pressure type air -line respirator with an integral auxiliary self-contained air supply of at least 1.5 minutes. a. Employees shall be instructed and trained in the use of self-contained breathing apparatus prior to exposure to IDlif atmospheres. b. No employee shall enter an DER atmosphere unless accompanied by another additional adequately protected employee or wearing a safety line and safety harness tended by a person in a safe area. The tender shall have no other duties and shall have the proper equipment available to assist he respirator wearer(s) in case of emergency. An effective communication system that will provide rapid notification of emergency response needs shall be maintained. 5. Atmosphere with Less Than 1.9.5% Oxygen. No person shall be permitted in. atmospheres containing less than 19.5% oxygen without wearing a Self -Contained Breathing Apparatus (SCBA) of an approved type, or a combination pressure -demand air -line respirator with an integral auxiliary self-contained air supply of at least 15 minutes. 3 o/ 5 6. Confined Spaces. All confined spaces shall be considered to be immediately dangerous to life or health (IDLH) until proven otherwise by testing. When self-containedbreathing apparatus or hose masks with blowers are .used in confined spaces or in atmospheres immediately dangerous to life or health, standby men must be present with suitable rescue equipment. Persons .using air line respirators in atmospheres immediately hazardous to life or health shall be equipped with safety harness and safety lines for lifting or removing persons from hazardous atmospheres or other and equivalent provisions for the rescue of persons from hazardous atmospheres shall be used. A standby man or. men with suitable self-contained breathing apparatus shall be at the nearest 'fresh air base for emergency rescue. In areas where the wearer, with failure of the respirator, could be overcome by a toxic or oxygen -deficient atmosphere, at least one additional rn.a.n shall be present. 7. Monitoring of Air Contaminants. The concentration of air contaminants and oxygen in the permit required confined space shall be monitored while personnel are inside the space. 8. Use of Respirators in Low -and High Temperature Environments. The use of respirators in low -and high —temperatures environments shall be in accordance with the requirements of ANSI Z88.2. 9. Anticipated Exposure Assessment and Air Monitoring. Based on previous similar jobs to this one, at the USARC/Central Mechanical Room 111, the anticipated worker exposure is less than lf/cc. a. initial Negative Air Assessment. Based on the previous similar job anticipated exposure, the workers will be using half -mask air purifying respirators equipped with high efficiency filters. This will give them of protection factor of more than 10 x b. Continuous Personnel Air Monitoring. One in every 5 workers inside the containment area shall be wearing a personnel pump with air filter cassette in the breathing zone for continuous air monitoring inside the working area. Results shall be posted at the entrance to .the work area within 48 hours of receiving them in the office. 10. Safety. The employer shall ensure that the respirator issued to .the employee exhibits the no fa.cepiece leakage and that .the respirator is fitted properly. Page .1. of 3 E. ..Medical Surveillance Guidelines 7 ROUTE OF ENTRY inhalation, ingestion TOXICOLOGY Clinical evidence of the adverse effects associated with asbestos, tremolite, anthophyllite, and actinolite, is present in the :fomiof several well-conducted epidemiological studies of occupationally exposed workers, family contacts of workers, and persons living near asbestos, tremolite, anthophyllite, and actinolite mines. These studies have shown a definite association between exposure to asbestos, tremolite, anthophyllite, and actinolite and an increased incidence of lung cancer, pleural and peritoneal mesothelioma., gastrointestinal cancer, and asbestosis. The latter is a disabling fibro tic lung disease that is caused only by exposure to asbestos. Exposure to asbestos, tremolite, anthophyllite, and actinolite has also been associated with an. increased incidence of esophageal, kidney, laryngeal, pharyngeal, and buccal cavity cancers, as with other known chronic occupational diseases, disease associated with asbestos, tremolite, anthophyllite, and actinolite generally appears about 20 years following the first occurrence of exposure. There are n.o known acute effects associated with exposure to asbestos, tremolite, anthophyllite, and actinolite. Epidemiological studies indicate that the riskof lung cancer among exposed. workers who smoke cigarettes is greatly increased over the risk of lung cancer among non -exposed smokers or exposed nonsmokers. These studies suggest that cessation of smoking will reduce the risk of lung cancer for a person exposed to asbestos, tremolite, antholphyllite, and actinolite but will not reduce it to the same level of risk as that existing for an exposed worker who has never smoked. Signs and sym.ptom.s of exposure -related disease the signs and symptoms of lung cancer or gastrointestinal cancer induced by exposure to asbestos, tremolite, anthophyllite, and actinolite are not unique, except that a chest t x- ray of an exposed patient with lung cancer may show pleural calcification, or pleural fibrosis. Symptoms characteristic of m.esothetioma include shortness of breath, pain on the walls of the chest, or abdominal pain. Mesothelioma is therefore more likely to be found among workers who were first exposed to asbestos at an. early age. Mesothelioma is always fatal. Asbestosis is pulmonary fibrosis caused by the accumulation of asbestos fibers in the lungs. Symptoms include shortness of breath, coughing, fatigue, and vague feelings of sickness. Page 2 of 3 When the fibrosis worsens, shortness of breath occurs even at rest. The diagnosis of asbestosis is based on a history of exposure to asbestos, the presence of characteristic radiologic changes, end -inspiratory crackles (rales), and other clinical features of fibrosis -producing lung disease. Pleural plaques and thickening may also be observed on x-rays. Asbestosis is often a progressive disease even in the absence of continued exposure, although. this appears to be a highly individualized characteristic. In severe cases, death may be caused by respiratory or cardiac failures. SURVEILLANCE AND PREVENTIVE CONSIDERATIONS As noted above, exposures .to asbestos, tremolite, anth.ophyllite, and. actinolite has been linked to an increased risk, of lung cancer, mesothelioma, gastrointestinal cancer, and asbestosis among occupationally exposed workers. Adequate screening tests to determine an employee's potential for developing serious chronic diseases, such as cancer, from exposure to asbestos, tremolite, anthophyllite, and actinolite do not presently exist. 111. The physician's opinion as to whether the employee has any detected medical conditions that would place the employee at an increased risk of material health impairment from exposure to asbestos. B.
