1 The parent must supply all medicines items that are to be given or used for the child. They will be kept in your
child's own medicine box so that there will be no confusion.
1 I do take "mildly ill" children. Those with mild coughs and colds have already exposed the other children before
they showed any symptoms and it is not necessary to exclude them from childcare, unless the child is
excessively irritable, persistent hacking cough or excessive runny nose and is in need of extra care. It will be
solely up to the provider if a child will be accepted into childcare, even if a doctor states they are able it come
back into the program. The provider has the right to refuse care if the provider feels that t would not be in the best
interest of the other families and staff in the program. The parent will be responsible to find alternative care.

Re: Revised letter pertaining Parking Spaces @ 537 NE 70 Street. Miami, FL.
This letter is being submitted as proof that Ms. Paula Vitale has contacted the Miami Parking Authority in
order to verify the number of parking spaces available for use by customers of this prospective business
to be located at 537 NE 70 Street. Miami, Florida. A review of the area confirmed that there a(e 2 spaces
readily available to the public at large and customers that might be visiting this new establishment. Ms.
Paula Vitale has requested for MPA to place a passing Loading Sign which will allow customers to park
Monday — Friday between the hours of 7:30 am — 5:30 pm for a maximum of 15 minutes. After reviewing
Ms. Vitale request MPA has agreed to place the sign as soon as all the requirements are met for the
Planning and Zoning Department.
