HANNON TYPE OF BENEFIT PROPOSED BENEFITS Effective November 21, 2013 SALARY $125,000 annual salary payable bi-weekly (To remain in effect until changed by the City Commission.) Car Allowance $300 per month Cellular Phone Allowance $100 per month INSURANCE Health/Vision/Dental Life Accidental Death & Dismemberment Insurance Disability Insurance Executive Benefit Plan 2X Annual Salary 2X Annual Salary In the event that a disability plan is offered to other City executives, the City Clerk shall be provided a similar benefit. EXECUTIVE PHYSICAL One annual physical LEAVE TIME Vacation Sick Holidays 20 days per year, with a maximum of 15 days to be taken consecutively. 10 days per year 11 days per year Upon honorable separation, the City Clerk shall receive payment of all Vacation and Sick leave earned as of the Effective Date of this Contract (earned Vacation and Sick leave will be paid out at the hourly rate associated with Mr. Hannon's position as Assistant City Clerk as of 1/14/2013). Vacation and Sick time earned after the Effective Date of this Contract that is not utilized during the Term shall have no "cash value." Such Vacation and Sick time shall be available on a use -it -or -lose -it basis over the duration of the Term. SEVERANCE NONE PENSION City of Miami Pension Plan. Participation in the Employee Pension Plan is authorized under Sec. 40-351 (3) of the Code.
