As Chairperson of the Evaluation Committee ("Committee") for the above services for the City of Miami, it is my
responsibility to offer the fmdings and recommendation of the Committee.
The City issued RFP 369325 for a Group Benefit Health Plan on July 12, 2013, and the four (4) proposals received
on August 22, 2013, were deemed responsive. The Committee, appointed by the City Manager, met on September
16, 2013, and was comprised of the following individuals:
1. Calvin Ellis, Director, City of Miami Risk Management (Chair)
2. Jair Espinoza, Group Benefits Manager, Risk Management, City of Miami
3. Jorge Davila, Assistant Risk & Benefits Officer, Miami -Dade County Public Schools
4. Rosa Novo, Fringe Benefits Supervisor, Miami -Dade County Public Schools
5. Margarita Mondejo, Personnel Benefits Specialist, City of Coral Gables
Following discussion, evaluation and deliberation of the four (4) proposals received, the Committee ranked the firms
as follows:
1. Cigna Health and Life Insurance Co. (447.90 points)
2. United Healthcare Services, Inc. (422 points)
3. Blue Cross and Blue Shield of Florida, Inc. (406.45 points)
4. Aetna Life Insurance Co. (403.35 points)
All five (5) committee members scored Cigna Health and Life Insurance Co.'s proposal as the top -ranked firm. The
Evaluation Committee recommends that the City enter into contract negotiations with the top -ranked firm, Cigna
Health and Life Insurance Co. The Committee further recommends that should contract negotiations fail with the
top -ranked firm, Cigna Health and Life Insurance Co., that the City negotiate with the second highest -ranked firm,
United Healthcare Services, Inc. Should contract negotiations fail with the second highest -ranked firm, the
Committee recommends that the City reject all proposals received and issue a new Request for Proposals.
Upon successful contract negotiations, the recommendation from the City Manager to the City Commission seeking
permission to authorize and execute the professional services agreement will be presented at the next available
meeting.
