INDIVIDUAL AS A MEMBER OF THE COMMUNITY TECHNOLOGY ADVISORY
BOARD FORATERMAS DESIGNATED HEREIN.

Section 1. The individual listed above is hereby appointed as a member of the Community
Technology Advisory Board, to serve for a term ending October 10, 2014, or until the nominating
Commissioner leaves office, whichever occurs first.
