Kemp, Chef Department of Fire -Rescue DATE: SUBJECT: February 26, 2013 Request Sole Source finding; Intermedix EMSystems; Fire -Rescue REFERENCES: ENCLOSURES: FILE : This memorandum serves to request that you approve a finding that Intermedix EMSystems, located at 135 South 84th Street, Suite #150, Milwaukee, WI, 53214, is the sole source provider for a multi- functional patient tracking solution, EMTrack, including the EMTrack Mobile MC75A Bundles utilized by the Department of Fire -Rescue. In the fall of 2009, the Florida State Department of Health purchased and began implementing a statewide patient tracking solution (DOH07-013 Communications and Patient Tracking System). The system called EMTrak was purchased to enable local Health Departments, First Responders, Fire and EMS Agencies, Hospitals and Disaster Management personnel to seamlessly capture patient information and communicate in large scale events, mass casualty incidents, evacuations and in daily patient encounters. Intermedix EMSystems is a recognized leading provider of comprehensive and interoperable technology solutions for emergency preparedness and response within the healthcare continuum. Intermedix EMSystems has significant expertise in the integration, deployment, and support of field hardware, software and communications infrastructure used for tracking people, patients, and evacuees. This experience ensures reliability and cost -savings in the specific selection of hardware, such to eliminate errors associated with alternate procurement channels. Intermedix EMSystems is confident that the combination of our proven track record, industry expertise, strong partnerships throughout the State of Florida, and numerous statewide implementations of communications and tracking solutions can provide a strong partnership to achieve the goals of the Florida regions and task forces. Page 2 The Department of Fire -Rescue has received a written quote from Intermedix EMSystems in an amount not to exceed $252,335.00, for EMTrack Mobile MC75A for the Urban Search and Rescue ("USAR") Task Force 2. It is therefore requested at this time, that the Chief Procurement Officer deem this acquisition as a sole source finding and recommend the acquisition purchase. Funds for this purchase will be allocated from UASI Account Code: #18-180026 -01.06.07 —1839 - Equipment —189000, for the Department of Fire -Rescue.
