Words and/or figures stricken through shall be deleted. Underscored words and/or figures shall be added. The remaining provisions are now in effect and remain unchanged. Please note the contents herein and reflect same on the documents you have on hand. On July 26, 2012 the IFB closing date and time was extended electronically via the Oracle System to Tuesday, August 7, 2012 at 1:00 PM. On August 3, 2012 the IFB closing date and time was extended electronically via the Oracle System to Wednesday, August 15, 2012 at 1:00 PM. The IFB Closing DatelTime has been changed to: FRIDAY, SEPTEMBER 14, 2012 AT 1:00 PM. The following is the City's answers to questions received from prospective bidders: 01. Where do we list the Price for Removal and Disposal of damaged EPDM Poured -in -Place, which should be priced based upon the Square Footage being removed and disposed? Al. All removal and disposal fees are to be included as part of the rate per square foot as bid. Section 3.3 of the IFB has been amended as follows: 3.3 CLEAN-UP Contractor shall: A. Be responsible for protection of finished surfaces until completion of construction and sign off. B. Not leave adhesive on adjacent surface or play equipment. Spills of excess adhesive shall be promptly cleaned. C. Properly dispose of all material and packing waste before leaving the job site. D. Be responsible for supplying a dumpster at job site for all waste associated with installation of the safety surface. All removal and disposal fees are to be included as part of the rate per square foot as bid. Q2. Where do we price the supply & installation of the following: i. EPDM Wear Cap only, priced per SF li. Poured -in -Place system: EPDM Cap and Basemat combined Addendum No. 2 - IFB 320278 1. 1.5" total thickness, priced per SF 2. 2.0" total thickness, priced per SF 3. 2.5" total thickness, priced per SF 4. 3.0" total thickness, priced per SF 5. 3.5" total thickness, priced per SF 6. 4.0" total thickness, priced per SF • Less than 50 SF • 51-99SF. • 100 -199 SF • 200 — 299 SF • +300 SF A2. Line Items 1-5 have been deleted and have been replaced with Line Items 6-40, as detailed in the Line Section of the IFB. Q3. I find no reference as to the color mixture of the EPDM Wear Cap. What's the color mixture you want us to quote: 50%StandardColor/50%Black? Or 100% Standard Color? Or 33.3%StandardColor/33%°StandardColor/33%Black? Or something else? A3. Section 3.1. of the IFB has been amended to reflect the following additional language: 3.1. SPECIFICATIONS/SCOPE OF WORK c) Surface shall consist of two layers. The base coat is recycled tire 100% wire free and a minimum of 1/2" EPDM rubber installed on compact ted crusted stone, concrete or asphalt. Color mixture of the EPDM Wear Cap shall be 50% Standard color/ 50% Black. Q4. How do you as the City of Miami address the following issue? b. If one of your suppliers isn't paying their material resources? (For example: If a company wins this bid with you for EPDM Poured -in -Place Repair Services and performs the repairs. However, they're not paying their material suppliers for the rubber and polyurethane. What recourse does the material supplier have within the City of Miami?) A4. The resulting contract from this IFB will be between the City of Miami and the Successful Bidder. In accordance with Section 2.7 (D), the Successful Bidder is required to have "sufficient financial support, equipment and organization to insure that they can satisfactorily provide the goods and/or services if awarded a contract under the terms and conditions herein stated." To comply with this requirement, the Bidder must obviously be in good relations with its material suppliers. Ultimately, it is the Successful Bidder's responsibility to meet all of its obligations with its suppliers. It is understood, however, that a Successful Bidder who continuously fails to meet its obligations with its suppliers, may find itself short of meeting requirements under this contract with the City. Therefore, if the Successful Bidder is unable to perform its duties according to the contract and submit a completed project free and clean of any pending lien notices or open permits, then the City of Miami reserves the right to find the Successful Bidder in default of meeting its contractual obligations, pursuant to Section 1.27 Default/Failure to Perform, Subsection C. ALL OTHER TERMS AND CONDITIONS OF THE IFB REMAIN THE SAME.
