HomeMy WebLinkAboutR-12-0446City of Miami Legislation Resolution: R-12-0446 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 12-01271 Final Action Date: 11/15/2012 A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S), ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER APPROVING THE FINDINGS OF THE EVALUATION COMMITTEE, PURSUANT TO REQUEST FOR PROPOSALS ("RFP") NO. 336312 FOR THE PROVISION OF AN EMPLOYEE BENEFIT DENTAL PLAN, AUTHORIZING THE CITY MANAGER, TO NEGOTIATE AND EXECUTE PROFESSIONAL SERVICES AGREEMENTS ("PSA"), IN SUBSTANTIALLY THE ATTACHED FORM, WITH THE TOP RANKED FIRM, CIGNA DENTAL HEALTH OF FLORIDA, FOR THE DENTAL HEALTH MAINTENANCE ORGANIZATIONS ("DHMO") PLAN; AND WITH THE TOP RANKED FIRM, THE GUARDIAN LIFE INSURANCE COMPANY OF AMERICA, FOR THE DENTAL PREFERRED PROVIDER ORGANIZATIONS ("PPO") PLAN, FOR THE DEPARTMENT OF RISK MANAGEMENT, FOR A PERIOD OF THREE (3) YEARS, WITH OPTIONS TO RENEW FOR TWO (2) ADDITIONAL ONE-YEAR PERIODS; ALLOCATING FUNDS FROM THE VARIOUS SOURCES OF FUNDS OF THE USER DEPARTMENTS, SUBJECT TO THE AVAILABILITY OF FUNDS AND BUDGETARY APPROVAL; FURTHER AUTHORIZING THE CITY MANAGER, SHOULD NEGOTIATIONS FAIL WITH THE TOP RANKED FIRMS, TO NEGOTIATE AND EXECUTE A PSA WITH THE SECOND RANKED FIRMS, THE GUARDIAN LIFE INSURANCE COMPANY OF AMERICA, FOR THE DHMO PLAN, AND WITH HUMANA, INC., FOR THE PPO PLAN; FURTHER AUTHORIZING THE CITY MANAGER, SHOULD NEGOTIATIONS FAIL WITH THE SECOND RANKED FIRMS, TO NEGOTIATE AND EXECUTE A PSA WITH THE THIRD RANKED FIRMS, METLIFE AND SAFEGUARD HEALTH PLAN, INC., FOR THE DHMO PLAN, AND WITH CIGNA DENTAL HEALTH OF FLORIDA, FOR THE PPO PLAN; FURTHER AUTHORIZING THE CITY MANAGER, SHOULD NEGOTIATIONS FAIL WITH THE THIRD RANKED FIRMS, TO REJECT ALL PROPOSALS AND ISSUE A NEW RFP SOLICITATION. BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recommendation of the City Manager approving the findings of the Evaluation Committee pursuant to Request for Proposals ("RFP") No. 336312 for the provision of Employee Benefit Dental Plans, that Cigna Dental Health of Florida, is the top ranked firm, for the Dental Health Maintenance Plan ("DHMO"), and that The Guardian Life Insurance Company of American, is the top ranked firm, for the Preferred Provider Organizations Plan ("PPO"), for the Department of Risk Management ("Risk Management"), is accepted. Section 2. The City Manager is authorized{1}, to negotiate and execute a Professional Services Agreement ("PSA"), in substantially the attached form, with the top ranked firm, Cigna Dental Health of Florida, for the DHMO Plan, for Risk Management, for a period of three (3) years, with the option to renew for two (2) additional one-year periods, with funds allocated from various sources of funds of City of Miami Page 1 of 2 File Id: 12-01271 (Version: 2) Printed On: 12/13/2017 File Number: 12-01271 Enactment Number: R-12-0446 the user departments, subject to the availability of funds and budgetary approval at the time of need. Section 3. The City Manager is authorized{1} to negotiate and execute a PSA, in substantially the attached form, with the top ranked firm, The Guardian Life Insurance Company of America, for the PPO Plan, for Risk Management, for a period of three (3) years, with the option to renew for two (2) additional one-year periods; with funds allocated from various sources of funds of the user departments, subject to the availability of funds and budgetary approval at the time of need. Section 4. Should negotiations fail with the top ranked firms, the City Manager is authorized{1} to negotiate and execute a PSA with the second ranked firms, The Guardian Life Insurance Company of America, for the DHMO Plan and with Humana, Inc., for the PPO Plan, for a period of three (3) years, with the option to renew for two (2) additional one (1) year periods, with funds allocated from various sources of funds of the user departments, subject to the availability of funds and budgetary approval at the time of need. Section 5. Should negotiations fail with the second highest ranked firms, the City Manager is authorized{1 } to negotiate and execute a PSA with the third ranked firms, Metlife and SafeGuard Health Plan, Inc., for the DHMO Plan and with Cigna Dental Health of Florida for the PPO Plan, for a period of three (3) years, with the option to renew for two (2) additional one (1) year periods, with funds allocated from various sources of funds of the user departments, subject to the availability of funds and budgetary approval at the time of need. Section 6. Should negotiations fail with the third ranked firms, the City Manager is authorized{1} to reject all proposals and issue a new RFP solicitation. Section 7. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{2} Footnotes: {1} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
