Please find attached requested information. I have included the new quote/contract for EMSAR Florida, proof of
insurance with proof of workers compensation, sole source letter and an overview of the Bronze service program. Please
let me know if City of Miami needs to be added as a certificated holder on our insurance.
You had a couple questions/ adjustments. with Medco's contract.
1. "Please remove the contract period and replace with this terminology:"
a. Medco's old contract will be voided since the company is under new management. All of our service
contracts are initiated as a one year contract and will automatically renew unless cancelled in writing
with 30 days notice. If City of Miami requires a new contract to be issued each year, we can provide this
information at the end of City of Miami's fiscal year for the next year's budget. Rates are locked in for
the life of the yearly revolving contract, unless EMSAR Florida provides written notice for the next year's
budget for approval. EMSAR Florida will provide advance written notice for all service increases. Rates
will not change mid -year. EMSAR Florida only uses or sells OEM approved parts and hardware. These
prices are dictated by the manufacture, not EMSAR Florida or EMSAR Inc.
2. "Are there any additional discounts you can supply to the City on the "per cot" repair rate of $63.00 and
$130.00", "Why does the repair rate on the 93P cot jump from $63 to $130 on the last year of the contract"
a. Your PM contracted rate for preventive maintenance is $80.00 per cot, a flat $50.00 travel for all 14
locations per quarter. $25.00 round trip travel charge and $90.00 per hour (1/4 hour minimum) for all
on -demand service or emergency repairs.
b. City of Miami will also receive an additional 5% discount for all accessories purchased by an EMSAR
Florida representative.
3. "Does the 5% possible increase apply to the per cot repair rate and parts?
a. The 5% increase from Medco does not apply with our new contract. EMSAR Florida's contract will
automatically renew unless terminated with a 30 day written notice from City of Miami or 90 day rate
increase notice from EMSAR Florida for the next year of service (not mid -year).
b. All part pricing is dictated by the OEM manufacture. 2012 price list are available upon request.
Please be aware, you will see a price decrease for the majority of parts sold by EMSAR Florida compared to Medco from
previous years. EMSAR Florida will only sell OEM parts at the manufacture's recommended list pricing.
