The following documents have been reviewed for formal competitive procedures and bid approval for
catered meals (afternoon snack):
• Bid advertisement from the publication of general circulation
• Bid opening log sheet
• Evaluation checklist for each caterer that submitted a bid
• Selected state menu (cycle menu A)
• Meal Services to be Provided, Attachment 5 included in bid packet
• Price schedule, Attachment 7 from each caterer that submitted a bid

• IFB evaluation of responses
• Addendum No.1
• Addendum No.2
• Bid security list
Your request to award the contract to a caterer with a higher bid is approved.
The Attachment 7, Price Schedule of the 2011-2012 Standard Catering Contract from Montoya
Holdings is calculated correctly -with a grand total amount of $150,469.80. This bid along with
supporting documentation is approved for acceptance of a 2011-2012 catering contract.
Upon execution of the catering contract by both parties, please make a copy of the signed contract
page (page 26) and fax to my attention at: 850-414-1622 or, scan and email to:
