ADDENDUM NO. 1
RFQ No. 287259 January 26, 2012
Request for Qualification for Demolition Services Citywide
TO: ALL PROSPECTIVE PROPOSERS:
The following changes, additions, clarifications, and deletions amend the RFP documents of the above captioned RFP,
and shall become an integral part of the Contract Documents. Words and/or figures stricken through shall be
deleted. Underscored words and/or figures shall be added. The remaining provisions are now in effect and remain
unchanged. Please note the contents herein and reflect same on the documents you have on hand.
The RFQ closing date and time has been changed to Thursday, February 2, 2012 at 1:00 p.m.
Section 3.7. SITE CLEANUP of the Specifications has been amended as follows:
Contractor shall provide proof that materials and debris have been recycled, as may be required and requested by the
Contract Administrator. Receipt from the recycling firm must be provided as proof.
Please find below Questions from prospective respondents and the City's Answers to those Questions received before
the stipulated due date:
Q1: The References you are requiring, do they have to be from the public sector. In other words, similar projects
performed for Miami -Dade County Unsafe Structures, SGA Dpt, Housing, or even for the City of Miami itself? Or
should they be from the "private sector", owners of residences, commercial buildings, etc. Or can it be a
combination of both?
Al: The City will accept references from both the Public and from the Private sector.
Q2: If the demolition is not a Community Development project and it is a residential demolition, will an asbestos
survey be required?
A2: If not a CD or CDBG project and the property is a single family or duplex and is demolished through general fund
dollars, no asbestos survey is required. If it is a commercial (three units or more) property, then DERM will
require the survey.
Q3: What do you consider satisfactory proof that the "materials and debris have been recycled'?
A3: Receipt from the recycling firm must be provided as proof
04: Do the Section 3 document has to be completed and submitted with the bid package.
A4: Yes, the Section 3 Plan, along with the rest of the forms included in Attachment C must be submitted with the
RFQ response. All other Section 3 documents/forms are submitted with reimbursement requests.
ALL OTHER TERMS AND CONDITIONS OF THE RFP REMAIN THE SAME.
