The following underlined revisions were made to the aforementioned resolution: 1. Sixth Recital WHEREAS, Choices et al Inc., will provide professional development to teacher/counselors and administrative staff on the Family Conferencinq Decision Making Model and provide onsite mentorinqduring a Family Conferencinq Decision Making Model session for each of the teacher/counselors or team members; and 5. Eight Recital WHEREAS Clerk of Courts for the Eleventh Judicial Court, will assemble and review case files for cases on the court calendar; ensure that Jury case files are complete for court action, obtaining missing documents as necessary; place unfiled documents in appropriate case files; and check court notices to verify if parties were notified; and 6. Ninth Recital WHEREAS, the Department is requesting for the City Commission to authorize the City Manager, after consultation with the City Attorney, to negotiate and execute Service Authorization Letters, Interlocal Agreements in accordance with Chapter 163.01, Florida Statutes, Professional Service Agreements , and any amendments, extensions, and modifications, as. applicable, in compliance with the related form of Trust Contract, respectively, with the designated Service Providers as follows: (a) The School Board of Miami -Dade County, for an amount not to exceed $522,861.42; (b) Choices et al, Inc., for an amount not to exceed $7,920.00; and (c) The Clerk of Courts for the Eleventh Judicial Court, for an amount not to exceed $3,164.25, each for the period from November 1, 2011 through July 31, 2012 with the option to extend for two (2) additional similar periods for the same not to exceed amounts, under the same terms and conditions; 7. We anticipate that all final contracts for this term of service will be executed on or before the end of February, however, we recognize that understandable delays in completing documentation may result in our not finalizing these contracts by end -January. All services reasonably performed through the time these continuing service authorizations are in effect will be considered for future reimbursement by subsequent invoice when the final budget is authorized, assuming that the City continues to comply with the requirements of the core contract including those involving insurance requirements, background screening, audits and eligible expenses. This requirement is no different than expressed previously in your service authorizations.
