Dear Ms. Fernandez:
On behalf of our client, Jokur Acquisitions, Inc., we hereby submit a public hearing
application for an amendment to the Miami Comprehensive Neighborhood Plan and a public hearing
application for an amendment to the Zoning Atlas and/or Overlay District (collectively. the
"Applications") in connection with the Property. The Property has been used as a parking lot in
conjunction with two lots to the west that are currently zoned C-1, consistent with the SD-12.
The purpose of these Applications is to assure that the properties are consistently zoned. and
request that the Property's land use designation change to Restricted Commercial, and its zoning
designation changed to C-1.

PUBLIC HEARING APPLICATION FOR AN
AMENDMENT TO THE ZONING ATLAS AND/OR OVERLAY DISTRICT
Welcome to Hearing Boards! This application is intended to serve as a guide in acquainting
you with our public hearing process. By any means, please feel free to contact us at the number
above, should you have any questions.
CITY OF MIAMI ORDINANCE NO. 11469, CODIFIED IN CHAPTER 2, ARTICLE VI OF THE CITY
CODE STATES THAT ANY PERSON WHO RECEIVES COMPENSATION, REMUNERATION OR
EXPENSES FOR CONDUCTING LOBBYING ACTIVITIES TO REGISTER AS A LOBBYIST WITH
THE CITY CLERK, PRIOR TO ENGAGING IN LOBBYING ACTIVITIES BEFORE CITY STAFF,
BOARDS, COMMITTEES AND THE CITY COMMISSION. A COPY OF SAID ORDINANCE IS
AVAILABLE IN THE OFFICE OF THE CITY CLERK (MIAMI CITY HALL), LOCATED AT 3500
PAN AMERICAN DRIVE, MIAMI, FLORIDA, 33133. Copies of lobbyist documentation with
proof of payment must be submitted with the complete application.
The deadline to file the complete application with supporting documents is the first five working
days (1-5) of each month from 8:00 am until 3:00 pm. The responses to this application must
be typed and signed in black ink. Please note that pursuant to Section 1304.2.2 of the Miami
Zoning Ordinance, no application shall be deemed to have been filed unless and until the
applications shall have been completed. All pertinent and accurate information/documentation;
i.e., the plans, reports, exhibits, shall be presented at the time of filing, in addition to the paid
receipt. The applicant is responsible for the accuracy of the information contained in the
application and all supporting materials. If you like, you could bring the materials to our office for
staff review before the deadline to ensure the application is complete. Upon submittal, this
application must be accompanied with a signed Zoning Referral.
You will be responsible, if needed, to bring an interpreter for the English language to any
presentation before city boards, committees and the city commission. A valid power of attorney
will be required if neither applicant or legal counsel representing the applicant execute the
application or desire to make a presentation before city boards, committees and the city
commission.
Copies of City Commission resolutions can be obtained at our website through the "Legislative
Hub", or for certified copies, contact the City Clerk's Office at 305-250-5360.
Applications for Zoning Change (Amendment to Zoning Atlas and/or Overlay District) and Land
Use Change (Amend the Comprehensive Neighborhood Plan) will not be accepted unless it is
signed or initialed and dated by the Planning Department designee.

A. Vicky Garcia -Toledo, on behalf of property owner Jokur Acquisitions, Inc
hereby apply to the Miami City Commission for approval in accordance with Article 22 of the
Miami Zoning Ordinance, and in support of that request, furnish the following:
1. Obtain signature or initial and date by the Planning Department designee prior to submittal.
2. Address(es) and folio(s) of property: 3663 SW 17 Street, Miami. Florida, bearing the folio
number 01-4109-029-0510
3. Two (2) 11 x17" original surveys of the property prepared by a State of Florida registered land
surveyor within six (6) months from the date of the application.
4. Present zoning designation(s): R-2
Statement as to why the present zoning designation is inappropriate.
The subject lot is part of an existing commercial use, and has specifically been used for parking
purposes in conjunction with said use. It is inconsistent and inappropriate to have different zoning
district regulations for unified lots, under the same ownership and address, which support the existing
commercial use. As a result, the existing district boundaries are illogically drawn in relation to existing
conditions on the property proposed for change, pursuant to Section 2210(f) of the City of Miami Zoning
Code.

Statement as to why the proposed zoning designation is appropriate.
The subject property is part of an existing C-1 use for approximately 25 years. The lot is
surrounded to the north and south by structures supporting apartment use and to the west by
commercial use.

7. At least two photographs that show the entire property (land and improvements).
8. A clear and legible copy of the recorded warranty deed and tax forms of the most current year
that shows the present owner(s) and legal description of the property.
9. A clear and legible copy of the subject property address and legal description on a separate
sheet of paper, labeled as "Exhibit A".
10. Affidavit and disclosure of ownership of the subject property (see pages 4 and 5).
11. Certified list of owners of real estate within 500 feet of the subject property (see pages 6 and 7).
12. What is the acreage of the project/property site? .42 Acres
13. Is the property within the boundaries of a historic site, historic district or archeological zone?

OF THE SOLE DIRECTOR
AND SOLE SHAREHOLDER OF
JOKUR ACQUISITIONS., INC.
The undersigned, constituting the sole member of the Board of Directors (the "Board")
and the sole shareholder (the "Shareholder") of JOKUR ACQUISITIONS, INC., a Florida
corporation (the "Corporation"), does hereby waive any and all requirements for notice of the
time and place, and any special purposes, of a meeting of the Board and Shareholder of the
Corporation and does hereby consent, pursuant to the Florida Statutes Sections 607.0821 and
607.0704, to the adoption of, and do hereby adopt, the following preamble, resolutions and the
actions specified herein;
WHEREAS, Bilzin Sumberg Baena Price & Axelrod LLP, through its
partner, A. Vicky Garcia -Toledo, Esquire P.A., ("Counsel") will be handling, on
behalf of the Corporation, certain documents and applications, including, but not
limited to, a public hearing application for an amendment to the Miami

authorizes, directs and empowers its Counsel to apply for, modify, execute and
deliver the Applications and any and all documents required in connection with
said Applications, on behalf of the Corporation;
FURTHER RESOLVED, that Joseph Kurstin, in his capacity as an
officer of the Corporation, be and he hereby is, authorized, empowered and
directed to take all such further actions and to execute and deliver all such further
agreements, instruments and other documents in the name and on behalf of the
Corporation, that he, in his sole discretion, shall deem to be necessary, proper or
advisable in order to fully carry out the intent and accomplish the purpose of the
