RECOMMENDATION
It is respectfully recommended that the City Commission adopt the attached resolution
authorizing the City Manager to execute an agreement, in a form acceptable to the City
Attorney, for Employee Health Plans with both Cigna for Third Party Administrative Services,
Prescription Drug Services, Employee Assistance Services and United Benefits/Companion
Life for Stop Loss Insurance. The agreements are both for 3 years with 2 options to renew for
two years each. The funding will be provided from Account Code 514401.623301.6.657.
BACKCROTJND
The City issued an RFP on April 5, 2004 for Employee Benefit Health Plan broken down into
4 sections: Part A — Third Party Administrative Services; Part B. — Stop Loss; Part C —
Prescription Drug Services; and Part D — Employee Assistance Program Services. Responses
for this RFP were due to the City Clerk by May 17, 2004. An evaluation committee made up
of both Union representation and City Administration analyzed the proposals received and
made a recommendation to the City Commission to allow the right to negotiate with the top
ranked firms. The City Commission authorized permission to negotiate with these firms on the
September 9, 2004 meeting. The attached agreements have been successfully negotiated to the
City's satisfaction and are ready for implementation.

As Chairperson of the Evaluation Committee ("Committee") for the above services for the City of Miami, it is
my responsibility to offer the findings and recommendation of the Committee.
The City issued an RFP on April 5, 2004 for Employee Benefit Health Plan broken dawn into 4 sections: Part
A - Third Party Administrative Services; Part B - Stop Loss; Part C - Prescription Drug Services; and Part D
- Employee Assistance Program Services. Responses for this RFP were due to the City Clerk by May 17, 2004.
The appointed Cornrnittee met and discussed the merits of the responsive and responsible proposals, and
evaluated each according to the Part(s) proposed, pursuant to the City's RFP. Following discussion, deliberation
and oral presentations, the Committee recommends the following Proposer(s), in rank order:
For the provision of Part A Services

2. Blue Cross/ Blue Shield
3. Avmed
For the provision of Part C Services
1. Cigna
2. Blue Cross/Blue Shield
3. Express Scripts
For the provision of Part B Services
I. United Benefits

We are requesting permission to negotiate with the number 1 firms in each part. Should contract negotiations
fail with the top ranked firm, the Committee recommends the City negotiate with the second ranked firm. If
negotiations fail with the second ranked firm, then permission to negotiate with the third ranked firms. 'On
Part II services only, if contract negotiations fail with the third ranked firm, then permission to negotiate with
the fourth ranked firm. The contracts will be negotiated until they are found to be acceptable by the City. Once
the contracts are negotiated, they wi1 brought bac .fore commission for final approval.
